Hello,

I found two lumps near the opening of my anus. My anus itches and burns after pooping. It started after I suffered diarrhea last month. I used Smuth cream, which relieved me, but now it started again, and the problem is recurring. Please give me a permanent cure. I can not bear this pain. It is very uncomfortable. Please help.

I could not see any lump or swelling, which you refer to near the anal opening. Please encircle the lump area in the picture to correlate better.

Secondly, you mentioned a burning sensation after defecation, and the problem started after you had episodes of diarrhea last month. The present situation after episodes of diarrhea does not fit any diagnosis. However, please answer a few questions so I can get to the root of the problem-

1. Do you have difficulty passing stool?

2. Is your stool hard?

3. Any complaints of constipation? And do you need to strain forcefully during defecation?

4. Any blood-tinged stool you have found earlier?

5. Is the lump painful on pressing and palpating? And does it interfere with defecation?

Hi,

No, I do not have any difficulty during defecation. I pass my stool freely without any problem except if I drink less water the previous day. My stool does not have any traces of blood. The lump is not painful or tender. Thank you.

I saw the picture (attachments removed to protect the patient's identity), and there is a swelling in the anal opening, which looks like hemorrhoid swelling.

I have the following suggestions for you-

1. You must make sure you do not strain during defecation. To avoid any kind of strain or hard stool, please maintain adequate hydration throughout the day.

2. You must take adequate fiber from salad and fruits.

3. To make stool soft, you can take stool softener syrup.

4. You must avoid spicy and fried foods.

5. You must regularly soak in a warm bath or sitz bath.

Place your anal area in warm water with drops of Betadine solution and soak for 10 minutes to 15 minutes two to three times a day.

6. Apply Anovate cream (Phenylephrine, Beclometasone, and Lidocaine) twice daily for a week only on the outer aspect of the anal opening to relieve swelling, pain, and itching.

7. If you do not get relief or if swelling is not reduced in the next ten days, I suggest you visit a local surgeon to get examined.

Do not continue the same cream for a longer duration as it also has side effects.
