My last menstrual period was two months back, and my hCG was 5900 IU/L. But no sac was found in the investigation, and nothing in the fallopian tubes. What does this mean?

The pregnancy is of just five weeks and one day as of date, and it is possible that the radiologist may have been unable to find it, even on a transvaginal scan neither in the uterus nor outside it. However, the positive hCG (human chorionic gonadotropin) levels indicate an ongoing pregnancy inside the body. You may get serial beta hCG levels done 48 hours apart to check for a 65 % rise in hCG levels, indicating a viable ongoing growing pregnancy. The scan may be repeated again after two weeks to localize the pregnancy.

I got my serial beta hCG done after 48 hours, which is 33790 IU/L. Before 48 hours, it was 5900 IU/L.

There is a definite rise of more than 65 %, and hence there is a definite ongoing viable pregnancy. You may repeat the ultrasound scan after ten days and see the gestational sac and even the fetal heartbeat.
