I am a 28-year-old who was feeling uncomfortable and came to know that my blood pressure was 170/108 mm Hg. So I went to the doctor, who advised me of the tests, and the results were Total cholesterol 205 mg/dL, HDL 67 mg/dL, LDL 136.5 mg/dL, Triglyceride 106 mg/dL, and Hemoglobin 16.2 gm %. Other reports like ECG, ECHO, ultrasound, sugar, urine, and kidney are normal. He advised me to use Telma 40 mg in the morning and Starpress XL 25 in the evening. He also advised me Rosuvas10 in the night. Now my problem is that I sometimes feel chest pain in my left hand daily after breakfast. Pain is mild but sometimes continues for even 30 minutes. I check my blood pressure regularly thrice daily and find 115/125 mm Hg and 80/90 mm Hg. The medicine mentioned above I am taking from the last ten days, but the pain is giving me anxiety. Should I continue my medicine without worry or go for further tests or an advanced stage? I am feeling well, and only this pain may be angina, is giving me anxiety. What are the possible causes of angina, and is there any limit to angina coming under serious situations? Please help me.

I understand your concern. I have gone through your reports and prescription. At your age, it is not very common to have cardiac disease. But due to familial or genetic causes, it can rarely happen. High blood pressure may have other secondary causes at this age which need to be evaluated. Now about your chest pain, it can be cardiac in origin or due to other causes like musculoskeletal or neurological. I would advise you to answer a few questions. Do you feel episodic palpitations or headaches? Do you feel weakness, tremors, or any history of unintentional weight loss? Is any family history of cardiac disease? Do you smoke or drink alcohol? Is any history of sudden dizziness or loss of consciousness? Please answer the above questions, and then I will advise you on further investigations to identify the cause of your high blood pressure at this early age.

Thanks for the reply. In the first conversation, I forget that my heart rate is always higher. In the last six months, I always find it around 85 beats per minute when I rest, and at the time walking or exercising, it reaches 120 beats per minute or sometimes more than that, but I never felt short of breathless or anything else, so I did not take it seriously. Now my answer to your questions as below. Sometimes, I feel episodic palpitations or headaches. No, I have not felt weakness, tremors, or any history of unintentional weight loss. Yes, my father expired at an early age, at 47 years, due to heart failure. It was his first attack, and within five minutes, he died. Yes, I smoke hookah and sometimes cigarettes but not daily. For example, hookah, I smoke at the weekend and cigarettes only two in a day but not daily. Last two-three years, I have been taking alcohol comparatively in quantity. Before that, I drank sometimes, and now I completely quit smoking or drinking after this problem. I never felt dizziness. I do not have any problems in my life. I am well settled, with no mental issues. One thing I want to share is that when I become free, my mind starts thinking so much about common things; hence it takes an extra load after some time, and I feel tired. Simply put, my mind always wants some pressure and work to do. It cannot rest, that is why I am a person with good knowledge, and my observation is so good that every known person takes advice from any work. I always try to learn every time, even for the small thing. I want to know every detail, and thus I feel exhausted. So this thing may give me a problem. Kindly advise.

I understand your concern. The medication you have been advised you can continue to take. There are a few tests that I would like you to get done. Do ECG (electrocardiogram), 24 hour urine metanephrines, USG (ultrasound) doppler renal arteries, Aldosterone/renin ratio, TSH (thyroid stimulating hormone test), serum cortisol level. Finally, it is great that you are enthusiastic and focused about what you do. But unfortunately, stress has become part of our life. So I want you to have a healthy lifestyle. Do 30 minutes of meditation daily with yoga exercises and make it your routine. Avoid smoking and alcohol consumption, as this would increase cardiac risk. Increase the serving of fruits and vegetables daily by at least three to four servings. Walk at least 30 minutes daily if you cannot do another cardio exercise. Avoid a non-vegetarian diet, fast food, and oily and fatty food. Please revert with tests or any other queries. I hope this information will help.
