Patient's Query

Hi doctor,

I was in a car accident two months back, and I was the driver. It was a hit and run. I have severe left leg, hip, and lumbar pain. The L2-L5 and C5-C6 are all previously fused. I have a spinal cord stimulator that has stopped working properly, but the paddles appear to be in the correct position at T7-T8. I no longer have coverage above the thighs. It was placed for lumbar pain 14 years ago. Since the accident, I have disc bulges in C7-T12, most of which are 2 mm. They have no mass effect on the cord and no neural foraminal narrowing.

Starting with T8, they are causing effacement of the epidural space with the abutment of both traversing nerve roots. The T10-11 broad-base protrusion is 4 mm with bilateral neural foraminal narrowing. My spinal cord stimulator no longer covers the same area. However, the laboratory representative does not believe the paddles are misplaced. In addition to leg pain, I also have a weakness. The pain in the lower legs are worse than upper legs, with the top of the feet feeling like they are on fire. Also, the right hip, rib, and lumbar region have pain. I had severe headaches for two weeks from whiplash, which is now subsided. Also, I have left butt pain near the fused SI joint but not at the SI joint. Finally, I have nerve pain down the left leg, mainly at the side of the knee. I am taking Ibuprofen and Medrol Pak. I do not want steroids or steroid injections. Kindly suggest me your opinion.

I understand your concern and would like to explain that the pain is related to multiple nerve compression in the lumbar vertebral column. Coming to this point, I would suggest adding Amitriptyline (tricyclic antidepressant) and Pregabalin (anticonvulsant and anxiolytic medication) to your therapy. Also, I like to suggest another combination of drugs, Duloxetine (Cymbalta) and Pregabalin. First, however, I suggest you to discuss the above treatment options with your doctor. And, there is no need for another surgery.
