Patient's Query
Hello doctor,
I am 26 years male, till now I consulted more than five HIV specialists, three pathologists, and four general physicians in the clinic, and they said your negative for HIV, do not test further but sir anxiety would not make me believe this, please clear my all doubts.
I had three protected sexual exposures ten years ago, one protected sexual exposure six years ago with FSW no sexual exposure, I was feeling good then, I developed COVID phobia and started symptoms like palpitations, weakness, burning sensation in hands, and low mood. So, I consulted psychiatrist and took medicine and I was fine for next two three months. But, in that period I lost weight 6.6 pounds. The next year, I think due to HIV, (I searched symptoms of HIV on internet). I am experiencing symptoms like burning sensation in hands, feet, legs, thighs, anywhere shifted from one place to another, weakness, low mood, no concentration on anything. So I tested myself for HIV with HIV rapid test kits, till now I have tested myself with five different manufacturers rapid test kits, and I tested using 40 rapid test kits and results are negative. Is this result conclusive after eight to nine years of sexual exposure? Kindly answer my following questions point wise, till now I consulted more than 10 doctors and I hope you will clear my all doubts.
1. Does a rapid HIV test detect HIV infection which was taken after nine years of sexual exposure?
2. Can HIV rapid test detect all types, subtypes, and strains of HIV? Are the tests that I performed accurate and conclusive? Am I HIV-negative 100 percent?
3. I tested with finger-pricked whole blood method, can HIV antibodies be in the same in amount as in whole blood, serum or plasma? Does whole blood give accurate results like serum specimens?
4. Sir, the above symptoms I mentioned are HIV last stage infection? I have been experiencing these symptoms for the last one and a half years. Do you think these are last-stage HIV symptoms. In the last three to four years I have not had long periods of fever, diarrhea, or something I only have the above-mentioned symptoms.
5. Sir, I fear testing in the lab because, I think that the result will change if I test in the lab. So, I continuously tested with home-based HIV test kits and in reference to my multiple HIV rapid test negative results appropriately 40 times with 40 test kits if I test in a lab with ELISA (enzyme linked immuno-sorbent assay) what will be the result?
6. Sir I am going through huge anxiety every day and these symptoms continue. I want treatment for this, so collecting the courage to test with ELISA please tell me what will be the ELISA result. Will it be negative or positive?
7. I performed an HIV rapid test with the manufacturer's instructions where 20ul of blood sample is needed. Some times I used to draw 20ul of the sample with a dropper, sometimes I used one drop of blood or half a drop of blood and the result came negative, is this correct procedure I followed?
8. I read somewhere that in the last stage of HIV, HIV antibodies are not detected. Is this true sir, I only have the above symptoms, not serious symptoms.
9. Sir if I am negative for HIV so why I am experiencing this symptom continuously?
10. I have attached my CBC report please look at it and tell me if you find any immunosuppressive, immunocompromised element in that report.
11. Sir please tell me with your whole experience do I need more test for HIV?
Kindly suggest.
Thank you.
Hello,
Welcme to icliniq.com
I understand your concern.
1. Once you are infected with HIV (human immunodeficiency virus) virus, the antibody stays in your blood for life long. The rapid cards are very sensitive and specific and they pick up positive at any point of time after infection. They are going to detect the antibodies against HIV 1 and 2 virus.
2. The rapid kits available are good enough to pick up infections by almost all strains of HIV. If you do not have an HIV infection, do not worry.
3. The whole blood gives good results comparable with serum and plasma. There would not be any change in antibody levels in whole blood compared to plasma and serum.
4. Rapid card results are similar to ELISA (enzyme-linked immunosorbent assay) nowadays. No need to undergo further tests. You are not suffering from HIV.
5. For now, there is no need to undergo ELISA tests. Just stay calm, relax, and enjoy your life. Do not get stressed up. Even if you are going to undergo ELISA, it is going to come as negative only. For your assurance, if you want, you can go ahead with the testing.
6. If your procedure is not right, the control band will not appear properly. Your control band is satisfactory in all the images you attached to this query. So, do not worry.
7. Late stage of HIV, you will be having full-blown AIDS with a severe form of illness. You will have all sorts of infections like meningitis and tuberculosis.
8. At one point in time, every human undergoes some kind of stress due to some issues. It could be something like post-sexual exposure, post-death of a closed one, any exam and related stress, and much more. It is up to us to handle the issues. Focus on some other work. Keep yourself busy, so that your mind would not be empty. Empty mind is the devil's workshop, so if you are not keeping yourself busy, your mind keep thinking something that is not good.
9. I am not able to see the complete blood count report. We can not comment on your immunity by seeing the CBC (complete blood count) report. For that we need to do a CD4 cell count which is not needed for you).
10. From my experience, I wanted to tell you these. Do not test further for HIV. Stay calm, stay relaxed, enjoy your life. Avoid unprotected sexual exposure with unknown persons in the future.
I hope this helps.
Kindly follow up if you have more concerns.
Thank you.
Patient's Query
Hello doctor,
Thank you sir for your quick response.
Please clear my below doubts point-wise:-
1. Did you see the all attached pictures of test kits that I performed at home?
2. I tested with finger-pricked blood, not with venous blood, so finger-pricked blood gives accurate results.
3. Sir for my above symptoms like a burning sensation in my body, weakness, pain in my legs, the low mood so I was taking tryptomer 10 mg tablet on my own 15 days ago, I took tryptomer for only six days but I am experiencing symptoms like irritation in the nose, pain in nose, headache, is this side effects of tryptomer tablet? I stopped taking this medicine but the symptoms persisted till now, I visited an ENT specialist in my city who examined me and told me it is an allergy and he prescribed me Okacet L and Deflawal tablet but after taking this medicine irritation in my nose persisted. How can I withdraw these symptoms? I also think that I am facing difficulty in breathing so I measured the Sp02 level in the oximeter it is 99%. Is it normal? Because of anxiety am I thinking that I have a breathing issue?
4. Sir can I consider myself 100% negative for HIV? If I test in the lab further with Elisa or Western blot or DNA, RNA PCR it will be negative with respective to my multiple times negative rapid test kits result?
5. Sir the above symptoms that I mentioned are related to anxiety/fear/depression? Can fear/anxiety cause this type of symptoms?
6. Some doctor says these are neurology symptoms but doctor I took pregabalin m75 tablet for one month, six months ago but no relief in those symptoms?
7. Sir last, what is your conclusion in my case, with your expertise, can I go to a psychiatrist for treatment for these symptoms?
8. Sir I read somewhere that some doctors told me that in the last stage of HIV, HIV antibodies are not detected, but the doctor I only have the above-mentioned symptoms, not had a persistent fever or diarrhea in the last four to five years. So what do you think the above-mentioned symptoms are last stage HIV symptoms like burning sensation in the whole body?
9. In the last sir, I was in continued anxiety and fear of HIV from the last two years and continued testing at home with home-based test kits, so Dr please tell me if is it 100% conclusive. Am I 100% negative for HIV? Do I need to test more?
10. I am an information technology working professional but due to fear of HIV and stress, I quit the job and cannot concentrate on anything, going through the worst time of my life.
Please help.
Thank you.
Hello,
Welcome back to icliniq.com
I understand your concern.
1. Yes, I saw all the tested kit images.
2A. Yes, finger-pricked blood gives accurate test results comparable with venepuncture blood.
2B. The symptoms are most probably due to some kind of allergy or can also be due to severe anxiety and stress.
3. Based on the history of sexual exposure, and rapid card test results, there is no evidence of HIV Infection. If you are going to test your blood by other methodologies like ELISA / CLIA / CMIA / PCR / Western blot, most probably it's going to be negative only, as there is no clinical evidence of HIV Infection (Lab tests are always an adjuvant to clinical diagnosis for confirmation of the diagnosis).
4. Yes, severe forms of anxiety and stress can cause these kinds of symptoms.
5. Anxiety and stress do not get controlled by pregabalin M 75. It needs yoga, and meditation to control your anxiety and stress. If needed, you might need counseling to control your anxiety and stress by a psychologist or psychiatrist.
6. You can visit a psychiatrist and undergo counseling to reduce stress and anxiety if you are not able to control with yoga and meditation. Most of the time, listening to good music, having a brisk walk, yoga, and meditation would be enough to control your anxiety and stress. I hope you will overcome these by yourself. Believe in yourself.
7. All your symptoms are not the symptoms of the late stage of HIV infection. In fact, the later stages of HIV infection are more cruel and present with life-threatening infections and malignancies.
8. Based on your history, and blood test details. You are not suffering from HIV infection. There is no need to undergo any further tests. Just stay relaxed and calm. Just enjoy your life. Take care of yourself and your loved ones. Take care of your nutrition. Involve yourself in other activities like playing, swimming, yoga, and meditation.
I hope this helps.
Thank you.
