Patient's Query

Hi doctor,

I found two spots on my stomach that are concerning me. They are round red bloody rings white wax-like in the middle and then a bright blue spot near or connected to it. I tried to clean off the blue spots thinking it may be something, but it does not come off. The area is quite tender and slightly raised. I took some photos. Please help.

Hello,

Welcome to icliniq.com.

Are you a diabetic? Do you have annular inflammatory lesions with elevated margins? Do you scratch or rub the lesion excessively? Do you have intense itching or spreading borders? Do you have a fungal infection in the past or a history of atopy, allergies, etc.? Attach the picture of the lesion for confirmation of diagnosis.

Patient's Query

Thank you doctor,

I am not diabetic, but I am on Metformin 500 mg BD to help with weight loss. I have eczema and mild asthma. I have only noticed these spots tonight, so I cannot comment if they have grown or their borders have spread. In terms of fungal infection, we suspected my cats might have ringworm and that I may have caught it as I had a single characteristic ring-like spot on my right hip (these spots are on my upper left abdomen) and so I have been taking Fluconazole 50 mg once daily for the past three weeks. The veterinarian did not find ringworm on them, so I kept taking it as a precaution. I am allergic to Metronidazole and Metoclopramide and sensitive to gluten and dairy. I am a fragile premutation carrier and was diagnosed last year with premature menopause. My daily medications are Fluoxetine 40 mg, Clonazepam 2 mg, Dexamphetamine 10 mg, Topiramate 50 mg, Metformin 500 mg, Femoston 2mg/10mg, Clonidine 100 mcg, Phenergan 25 mg, Quetiapine 200 mg.

Hello,

Welcome back to icliniq.com.

The lesion visible in the image (attachment removed to protect patient identity) are hives (urticaria) and edematous inflammatory lesions with secondary bacterial infection (probably due to cat scratch or injury). Take tablet Zental 400 mg stat (immediately) only once, tablet Roxid 150 mg twice daily after food for five days, tablet Loratadine 10 mg after breakfast for ten days, tablet Levocetirizine 5 mg at night after dinner for seven days. Apply Calamine lotion twice daily for ten days (on edematous inflammatory lesions), Fucidin cream twice daily for seven days (on ulcer-like lesions). Avoid allergens like dust and pet contact. You can use gentle cleansing lotion like Cetaphil lotion after a bath for 20 days. Also, avoid scratching or rubbing of lesions. Do you have a fever or joint pain?

Patient's Query

Thank you doctor,

I have had a fullness sensation in my head with a post nasal drip for several weeks. Anytime I lie down and then get back up, I have the drip that I have to cough up, and I get a thick yellow/green clump. I also experience some hearing loss and fullness in my ears. The fullness in the head usually settles somewhat after an hour or two of sitting upright. I get intermittent headaches, mild fevers, joint pain (often), and severe fatigue. I have very poor temperature regulation and extreme body heat.

Hello,

Welcome back to icliniq.com.

Take tablet Diclomol (Diclofenac and Paracetamol) once a day (if you have pain or fever) for three days. Eat healthy and nutritious food. Drink lots of water and stay hydrated. Avoid stress and take tablet Limcee (Vitamin C) 500 mg once a day for 30 days (to heal lesions faster). Stop anti-fungal treatment, apply Fucidin cream twice daily in the morning and night, or Nadibact cream twice daily for seven days (on ulcer-like lesions). Follow up after seven days.
