Hi doctor,

My blood test report shows TSH (thyroid-stimulating hormone) level as 3.2 mIU/mL and FT4 (free thyroxine) at 0.75 ng/dL. My doctor told me it was normal. But I suspect hypothyroidism because a study shows that 95 % of the population has a TSH range of 1 to 2.5 mlU/mL. I feel depressed without reason, and I cannot tolerate cold weather. Should I eat tablets of Levothyroxine due to fewer side effects? I have been taking SSRI (selective serotonin reuptake inhibitor) like tablet Escitalopram for the past four weeks with tablet Methylphenidate for the past 15 years.

From the reports attached (attachment removed to protect the patient's identity), your thyroid profile looks normal. So do not take any thyroid medication. According to the international guidelines, the TSH level should be more than 10 mIU/mL with decreased T3 (triiodothyronine) and T4 (thyroxine) levels to necessitate thyroid replacement therapy. Also, unnecessary medications can cause cardiovascular complications. The probability of hypothyroidism is relatively low in males. So your fatigue and cold intolerance can be non-thyroid-related conditions like dehydration, vitamin D or magnesium or potassium deficiency, lack of physical activity, a diet lacking carbohydrates and fats, poor sleep hygiene, alcohol, and fatty liver. Identifying the cause and treating the same helps in improving the symptoms.
