Hi doctor,

My partner had sex with me before ten days. During that time, my condom was out and went inside of her without our knowledge. I did an ejaculation, and so she took one I-Pill on the evening. Now, we are worried about unwanted pregnancy. She says that her breasts have become big and her nipples are giving pain. Is this a symptom of pregnancy? Will that one I-Pill works? Can she take one more I-Pill now? Please advise us.

If she has taken I-pill (Levonorgestrel) in a proper dose within 48 to 72 hours of unprotected sex, without any vomiting, then it will work, and she will have bleeding within one to two weeks. Please mention about the menstrual history. It would be better, and I can guide you further. If it was not a fertile period of her cycle, then the chance to have pregnancy is less. You can check serum beta hCG (blood pregnancy test) if she does not get withdrawal bleed in three to four days. Due to externally induced hormones, she can have engorged breast. Externally induced hormone interferes with the periods to prevent conception by stopping fertilization, ovulation or implantation. I-pill is beneficial in the first 48 to 72 hours of post unprotected penetrative sex. It is already a week. She will have bleeding in the coming days if the I-pill is taken in a proper dose.

According to the details, if she has a 28 days cycle, then it could be a fertile period when you had sex. In the fertile period of the cycle, the chances of having pregnancy are more. But if she has taken I-pill in the proper dose, within 48 to 72 hours of sex and without vomiting, then she should have withdrawal bleeding in three to four days if the body responds well. But if she is unable to have bleeding, then check serum beta hCG to know the pregnancy status. If she gets withdrawal bleeding, it means that the pill has worked.

She has not got bleeding. So, I decided to check the serum beta HCG test. If the test is positive, what can I do? We are not ready for pregnancy. Do we need to wait for few more days for bleeding? Or it needs to be tested immediately?

If I-pill is taken in a proper dosage, then there is no need to take any other medication. If beta HCG comes negative, then there is no need for taking medication. I-pill can disturb cycles, and for withdrawal bleed, in case of negative pregnancy test, she can have tablet Primolut N 10 mg (Norethisterone) twice daily for five days to have a withdrawal bleed in one to two weeks.
