How to manage asymptomatic infectious diseases?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I recently tested positive for Candida albicans, Ureaplasma urealyticum, and Gardnerella vaginalis. I have not taken any medication yet, and I do not have any symptoms. I need your help with a treatment plan.

Please advise.

Hello,

Welcome to icliniq.com.

Thanks for providing a thorough medical history; it really helps ensure you receive the safest and most effective guidance.

Although you do not have any symptoms, testing positive for Candida albicans, Ureaplasma urealyticum, and Gardnerella vaginalis may warrant treatment in specific situations. Let us break it down with a personalized overview:

1. Candida albicans (yeast infection):

Treatment is only necessary if you have symptoms, such as itching, unusual discharge, redness, or discomfort. No symptoms? No antifungal treatment is needed at this time. If symptoms develop later, you can take Fluconazole 150 mg orally, a single dose, or a topical antifungal (Miconazole) for local relief. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

2. Ureaplasma urealyticum:

This may require treatment in certain situations, even if symptoms are not present. Treatment is generally considered if you experience abnormal vaginal discharge, a burning sensation during urination, pelvic pain, or if you are facing infertility issues. It is also recommended if you are currently pregnant or planning to conceive, as well as if you have had multiple sexual partners or if your partner is symptomatic or has tested positive.

If treatment is deemed necessary, the standard options include Doxycycline 100 mg taken orally twice a day for seven days, or Azithromycin 1000 mg taken orally as a single dose. An alternative Azithromycin regimen involves taking 500 mg on the first day, followed by 250 mg once daily for the next four days. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

3. Gardnerella vaginalis (bacterial vaginosis):

Treatment is only recommended if you have symptoms, such as fishy vaginal odor, thin, grayish discharge, and irritation or discomfort. If symptomatic, you can take Metronidazole 500 mg orally twice a day for seven days. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

Recommended plan (if still asymptomatic):

  1. No treatment is needed at the moment.
  2. Avoid douching or scented vaginal products.
  3. Practice safe sex (use condoms).
  4. Encourage your partner to get tested.
  5. Monitor for symptoms and consider retesting in six to 12 weeks or sooner if symptoms arise.

Important notes:

  1. Even if you are symptom-free, some clinicians choose to treat Ureaplasma in cases involving fertility concerns or recurrent infections.
  2. If you are planning a pregnancy or feeling anxious, it is reasonable to discuss preventive treatment with your gynecologist.

I hope this information is helpful.

Thank you.
