Patient's Query

Hi doctor,

Seven days back, I had got two dental implants. Two days after the surgery, the tissue around one of them became red, and I had severe pain. I took oral antibiotics, went to the doctor, who did an x-ray of the implants and said everything was fine with the implants. But, the inflammation did not go away even after three days of taking antibiotics. Now, the pus discharge is also there from the wound.

I have been taking the tablet Clindamycin 600 mg for three days. After my surgery, for the first four days I had taken the tablet Clindamycin 300 mg.

Hi,

Welcome to icliniq.com.

I am analyzing your report and attachments (attachments removed to protect the patient's privacy).

I will send you the details shortly.

Patient's Query

Hi doctor,

Thank you for your response.

Do you think I should do anything else with this inflammation besides taking the oral antibiotics?

Thank you again.

Hi,

Welcome back to icliniq.com.

I checked your attachments (attachments removed to protect the patient's privacy). There is definitely an inflammation, which is most likely due to an infection around the implant.

I also discussed this case with a renowned implantologist here, and I advise you on the following medicines:

1. The injection Amoxycillin and Clavulanic Acid (1.2 grams) twice a day (intravenously) for five days.

2. The tablet Metronidazole 400 mg thrice a day for five days.

3. The mouthwash Hydrogen Peroxide thrice daily for 15 days.

Let us wait and watch for five days. If the symptoms subside, then there is no need to worry. Otherwise, you need to physically consult a dentist.

However, if you are allergic to the above-prescribed medicines, then do tell me.

Please send an x-ray of these implants again, if possible.

Patient's Query

Hi doctor,

Thank you for your response.

I do not have an x-ray. It is with my dentist, but yesterday I visited him again. He said that the inflammation is not there near the implant, nerves are fine, bone is healthy; I should just continue with the antibiotics, and it will be completely fine.

The antibiotics you suggested, do they need to be injected, or can I take them orally also?

Hi,

Welcome back to icliniq.com.

The prescribed medicines are injectable (1.2 grams injection). But, if you are not comfortable with them, then you can take one gram tablets of the same antibiotic twice daily for five days.

Patient's Query

Hi doctor,

What if I continue with the tablet Clindamycin and add Amoxycilin injections? I have it at home already, and I can start it right now.

Thank you.

Hi,

Welcome back to icliniq.com.

No, I think you should continue with the tablet Amoxycillin and Clavulanic Acid, a broad-spectrum antibiotic.

Combining two antibiotics may result in resistance due to overdosage.

Patient's Query

Hi doctor,

I cannot go to the dentist for the next seven days, and because of that, I am asking again. Here we have just three antibiotics available: the tablet Azithromycin 500 mg, the tablet Cephalexin 500 mg, and the tablet Augmentin. So, the tablet Metronidazole that you suggested I cannot get for the next five days.

Can I make any combination of the above-mentioned drugs?

Thank you.

Hi,

Welcome back to icliniq.com.

The tablet Augmentin is fine, and it is the same as I had mentioned. Try it for five days and also the tablet Metronidazole 400 mg.

Patient's Query

Hi doctor,

I cannot go to my dentist for the next seven days. Here we can get further antibiotics: the tablet Azithromycin 500 mg, the tablet Cephalexin 500 mg, the tablet Augmentin. So, I cannot get the tablet Metronidazole, as you suggested. Can I make any combination with the available antibiotics that I mentioned above?

I am still taking the tablet Clindamycin for the last seven days.

Thank you so much.

Hi,

Welcome back to icliniq.com.

You can go with the tablet Augmentin only. There is no need to take other medicines. As I mentioned, you will be fine in five days.

Patient's Query

Patient's Query

Hi doctor,

It is becoming better but my day does not pass without taking a painkiller. I take the tablets Diclofenac, Ketoprofen or Ibuprofen. I found the tablet Ketoprofen to be the strongest. It started bothering me last week which was the ninth day of my antibiotic therapy and I became scared. The worst is at night. There is a fear of pain and I am not able to sleep until morning. It has been like this for the last seven days. Every day, I spend thinking that night will be better but it is not. I had never experienced anything similar before so I am concerned, but the x-ray looks fine. I started taking the tablet Augmentine yesterday and hope that it will help me.

Hi,

Welcome back to icliniq.com.

I understand your concern.

You will be fine in a couple of days. The pain is due to the bone drill and the infection around it. Do not worry.

I hope this helps you.

Same symptoms don't mean you have the same problem. Consult a doctor now!
