Why does the inflammation around the dental implant persist?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

Seven days ago, I had two dental implants placed. Two days after the surgery, the tissue around one of them became red, and I had severe pain. I took oral antibiotics and went to the doctor, who did an X-ray of the implants and said everything was fine with the implants. However, the inflammation did not resolve even after three days of antibiotics. Now, there is pus discharge from the wound.

I have been taking the tablet Clindamycin 600 mg for three days. After my surgery, for the first four days, I took the tablet Clindamycin 300 mg.

Please help.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

Should I do anything else for this inflammation besides taking the oral antibiotics?

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

I checked your attachments (attachments removed to protect the patient's privacy). There is definite inflammation, which is most likely due to an infection around the implant.

I also discussed this case with a renowned implantologist here, and I advise the following medicines:

  1. Injection of Amoxycillin and Clavulanic acid 1.2 grams twice a day intravenously for five days.
  2. Tablet Metronidazole 400 mg three times a day for five days.
  3. Use Hydrogen peroxide mouthwash three times daily for 15 days.

Let us wait and watch for five days. If the symptoms subside, then there is no need to worry. Otherwise, you need to consult a dentist in person.

However, if you are allergic to the above prescribed medicines, please inform me.

Please send an X-ray of the implants.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

I do not have an X-ray. It’s with my dentist, but I revisited him yesterday. He said that the inflammation is not present near the implant, the nerves are delicate, and the bone is healthy. He advised me to continue with the antibiotics and said that it would be completely fine.

The antibiotics you suggested, do they need to be injected, or can I take them orally as well?

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

The prescribed medicines are injectable 1.2-gram injections. However, if you are not comfortable with them, you can take one-gram tablets of the same antibiotic twice daily for five days.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

What if I continue with tablet Clindamycin and add Amoxicillin injections?

I already have them at home, and I can start them right now.

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

No, you should continue with the tablet Amoxicillin and Clavulanic Acid, a broad-spectrum antibiotic.

Combining two antibiotics may result in resistance due to overdosage.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

I cannot go to the dentist for the next seven days, so I am asking again. Here we have only three antibiotics available: tablet Azithromycin 500 mg, tablet Cephalexin 500 mg, and tablet Augmentin (Amoxicillin+Clavulanate). Therefore, I cannot get the tablet Metronidazole that you suggested for the next five days.

Can I make any combination of the above-mentioned drugs?

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

Tablet Augmentin (Amoxicillin+Clavulanate) is fine, and it is the same as I mentioned. Take it for five days along with the tablet Metronidazole 400 mg.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

Here, the available antibiotics are tablet Azithromycin 500 mg, tablet Cephalexin 500 mg, and tablet Augmentin. Therefore, I cannot get Metronidazole in tablet form as you suggested.

Can I combine any of the antibiotics I mentioned above?

I have been taking Clindamycin tablets for the last seven days.

Please help.

Thank you.

Hello,

You can take the tablet Augmentin (Amoxicillin + Clavulanate only). There is no need to take any other medicines. As I mentioned, you should be fine in five days.

Patient's Query

Hi doctor,

Thank you for the reply.

It is becoming better, but my day does not pass without taking a painkiller. I take tablets of Diclofenac, Ketoprofen, or Ibuprofen. I found tablet Ketoprofen to be the strongest. The pain started bothering me last week, which was the ninth day of my antibiotic therapy, and I became scared. It is worse at night. There is a fear of pain, and I am not able to sleep until morning. It has been like this for the last seven days.

Every day, I hope the night will be better, but it is not. I have never experienced anything similar before, so I am concerned, but the X-ray looks fine. I started taking the tablet containing Amoxycillin and Clavulanic Acid yesterday, and I hope that it will help me.

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

You will be fine in a couple of days. The pain is due to the bone drill and the infection around it. Do not worry.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
