Hi doctor,

I am a 23-year-old male weighing 176 pounds. I have a recurring infection in my right testicle that has been cleared twice with antibiotics. The first time was a course of Ofloxacin for seven days which relieved, but there was a dull and constant pain. And it came back after two weeks. I then took the same antibiotic for 14 days which cleared the problem. I did not have the symptoms for the past two months but the pain has come back, and the testicles are slightly hard and enlarged. I have been with the same partner for two years, and I am sure it is not an STI. I managed to get a prescription for Levofloxacin 750 mg, and the pharmacist had only three pills left and said the course was sufficient. However, I am aware of this problem's recurring nature and do not want to take a course that is too short. Given the side effects associated with Levofloxacin, if it does not clear the infection entirely, what is the recommended course for this antibiotic? Kindly give me your suggestions.

This seems to be prostatitis. I will suggest a few medications, and during the course, frequent ejaculation is advised thrice weekly. Please do not engage in any sexual activities during the treatment. I suggest you to take antibiotic Ciprofloxacin 500 mg twice daily for four weeks, tablet Tamsulosin (alpha-blocker) 0.4 mg once at night for a month, and take supplements like Prostafree capsule (health supplement) once daily for three months.

Thank you for your reply.

I do not feel comfortable taking a completely different antibiotic. I am aware that Ofloxacin and Levofloxacin are in the same group of antibacterial medications. Therefore I feel this is a safer and more effective option for the time being. What is the minimum recommended course? Please suggest.

The prostate requires two to six weeks of antibiotic treatment, but we recommend four weeks. The prostate has a capsule, and the blood circulation and penetration are complex. So antibiotic needs more prolonged exposure.
