Hello doctor,

I am a 20-year-old guy, and I find it hard to concentrate. I also have had forgetfulness, loss of interest in everything, and difficulty handling numbers for the past year. The problem I am facing is that I am having intrusive sexual thoughts about some of my family members. I do not even have any intention of having such thoughts, but still, my brain forces me to think so. Whenever I pass by them, I feel so nervous about facing them. Because of this, I am too much depressed. In the past three months, I lost most of my hair. I also lost interest in others (which I do not have since my childhood itself). I do not feel like eating or even drinking water. My mood swings are high. I do not even know what to say to others when they are with me. I try hard to start a conversation to keep them interested. I even expect more online messages from my friends even though I do not get one. If I feel interested in something after some time, I will lose interest in them all of a sudden. I am sleeping a lot. Always getting stressed about things that once did not bother me. Expecting love from even people I do not know much. My friends' circle became too small. I am afraid this will get me into absolute loneliness. Even medication was not helpful. Please help me.

I thoroughly read your query and understand your concern.

You most probably be suffering from obsessive-compulsive disorder (OCD). In this disorder, the patient has intrusive, uncontrolled, irrational images or thoughts which come despite the patient not wanting to feel that way. Though a proper assessment is needed, OCD often leads to secondary depression, and you are probably suffering from the same. I would advise you to seek help for assessment and treatment. I would be happy to help. Suggested treatments include medicine and behavior therapy both. I suggest you take the following investigations to be done, s-TSH (thyroid-stimulating hormone sensitive), and I would suggest you start a medicine I am prescribing you.
