Hello doctor,

I am a 21-year-old female. I got my first period at the age of 12. My periods have always been irregular, I get my period only two to three times a year. I am currently on my period and this time the flow is very heavy. I am taking medication for depression, tablet Zoloft. I was wondering what may be the cause for this?

I have read your query and understand your concern.

You are still young, and you may experience anovulatory cycles because of the underdevelopment of the reproductive system. However, with time the hormonal system develops, and you will have proper ovulatory cycles.

I will suggest you have the following medications-

  • Tablet Primolute 5 mg thrice daily from the fourth to 24th day of the menstrual cycle. This will help regulate the cycle.
  • Tablet Ponstan 500 mg and tablet Tranexamic acid 500 mg every eight hours for three days if the bleeding stops. You can take the tablets for five days if the bleeding does not stop.

In the next period cycle, on the second day of your period, you will have to undergo the following blood tests-

  1. Serum testosterone and serum progesterone.
  2. LH (luteinizing hormone) and FSH (follicle-stimulating hormone).
  3. Insulin and Prolactin hormone levels.
  4. Thyroid function tests.
  5. CBC (complete blood count).
  6. I suggest you carry out a transvaginal scan when you are not bleeding.

Follow the instructions mentioned below to regulate your menstrual cycle naturally-

  • Have a healthy balanced diet. Avoid food that is oily and spicy. Reduce the intake of salt and sugar in your diet. Drink plenty of water and always be hydrated.
  • Exercise regularly, and indulge in healthy recreational activities like swimming, dance, and yoga.
  • Maintain a healthy weight according to your BMI (body mass index).
