Hello doctor.

I am a 24-year-old female. I weigh 133 pounds, and I am five feet tall. I have PCOS. I got my period only once in the past year, and it lasted for three days and was light. It has been six months since my last period, and I have not had another till now. I also suffer from leg cramps, headaches, anxiety, depression, facial hair growth, dark spots, skin infections, insomnia, bloating, redness, and swelling of my hands. I have never had such symptoms before. In addition, I have constantly been gaining weight, and it is difficult for me to lose weight. I crave carbohydrate-rich foods, and I get anxiety attacks due to that.

Currently, I am taking one capsule of Dicyclomine 10 mg three times daily and one tablet Metformin hydrochloride extended-release 500 mg every day. I have checked my CMP and the results showed high BUN (22 mg/dl) and a high level of total protein (8.7 mg/dl). What should I do?

I understand your concern. Your symptoms are related to polycystic ovarian syndrome (PCOS). I suggest you to follow a few instructions mentioned below.

  1. Consult a nephrologist regarding the high protein and BUN (blood urea nitrogen).
  2. Take tablet Norethisterone 10 mg twice daily for seven days. You will get your period in a week. Kindly consult a specialist doctor, talk to them and take the medication with their consent.
  3. On day two of the menstrual cycle, test serum testosterone, insulin, LH (luteinizing hormone), FSH (follicle stimulating hormone), progesterone, and prolactin.
  4. Quit recreational drugs.
  5. Do recreational activities like swimming and painting.
  6. Exercise regularly.
  7. Go for walks and do yoga.
  8. Reduce weight.
  9. Have a nutritious, balanced diet but with less sugar, salt, oil, and spice.
  10. Make sure you get treated for any underlying medical problems that you have.
  11. Manage facial hair growth with beauty treatments like waxing and threading.
  12. Use homemade masks with honey, rose water, and rice flour or mud masks for skin problems. Do not use any chemical beauty treatments.
  13. Avoid taking unnecessary hormones unless prescribed.
  14. Avoid taking food after eight at night.
  15. Drink lots of water daily.
  16. Consume more vegetables, fruits, and fish.
  17. Avoid eating red meat and chicken.
  18. Follow a healthy lifestyle to stay healthy.
  19. Follow-up with the laboratory results as suggested.
