Q. How to treat the itchy skin itchy skin near my penis and anus?

Hi doctor,

Besides my penis and toilet side, some skin problems happened, and I am also experiencing itching. I have applied Soframycin cream for the last five days but did not get any relief. I have attached images so it will be easier to know. Please help.

I understand your concern.

As per the history and images provided (the attachments are removed to protect th patient's identity), it may be Tinea cruris. Tinea cruris is a fungal infection most often seen on the folds and private areas where exposure is less. They appear due to stagnation of sweat and moisture. Private areas, groin, and buttock areas are major skin fold areas that would be affected. Underarms, under the abdomen, and folds of arms and legs are other areas to be involved. They may also form due to decreased immunity, spread from an infected person, hygiene issues, climate changes, and hormonal changes. They present with itching, sometimes oozing and foul-smelling secretion, pain, and redness. Itching is the most common feature, leading to discomfort throughout the day, especially when sweating.

I suggest,

1. Ketoconazole soap to be used for bathing twice daily. Lather the soap with water and apply it to affected areas only. Keep for three minutes and then wash off. Do not use this soap on unaffected areas. Instead, use regular soap on other sites of the skin.

2. Terbinafine cream twice daily application. After bathing, clean the areas well, towel dry, air the areas to get rid of moisture, and apply Terbinafine cream. Make sure to wash your fingers well after applying the cream.

3. Terbinafine dusting powder. Apply on the affected area in the afternoon and evening to get rid of itching and absorb moisture. Twice daily application is advised.

4. Capsule Itraconazole 200 mg once daily at night after food for four weeks. It is an antifungal capsule.

5. Capsule Becosules Z ( vitamin B complex, zinc, and vitamin C) to be used in the morning after breakfast for one month. It helps with restoring immunity.

6. Avoid stagnation of moisture and sweat. Wipe out in case of sweating and air the area well under a fan from time to time.

7. Wear loose clothing and underwear. Make sure to wash your used belongings and dry them under the sun daily.

8. Do not share your belongings, and make sure any of your family members, if affected, need to get treatment to prevent recurrences.

Tinea cruris will take time to reduce and fade off due to the extent of the infection and the location being unexposed areas. It may take from weeks to months to lessen in severity and fade. Therefore being consistent and patient with the medication is essential.
