As per the history and images provided (attachments removed to protect the patient's identity), it is an insect bite reaction (IBR) due to bug bites. They appear as itchy, red to dry papules with increased itching sensation toward night time, most often acquired from insect bites from bed. They appear all over the body, including folds and private areas. They are treatable and therefore do not worry about the same.
My advice:
1. Permethrin 5 percent lotion to be applied from neck to down all over the body (neck to toe, including the web spaces between fingers and toes) but avoid face and head. Keep for 24 hours without washing. After 24 hours, wash off the lotion with the below soap. Repeat the same process after one week.
2. Permethrin soap to be used to wash off the lotion. To be used for one week until the second option application and then stop its usage and may switch to regular soap.
3. Tablet Ivermectin 12 mg single dose at night one hour before sleep. And repeat by taking one more tablet after one week, one hour before going to sleep at night.
4. Tablet Cetirizine (Cetrizine Hydrochloride) 10 mg one hour before going to sleep for one week. Take one tablet daily. Do not drive or go out after consuming the tablet, as it may cause sedation.
5. Avoid sharing belongings; in case of any other infected adult family member, they may undergo immediate treatment to avoid recurrences.
6. Wash your clothes in hot water, including those worn during the treatment, bedspreads, and pillowcases. After washing them well, dry them out in the sun, and once dry, store them in a plastic bag for seven days, after which they may be reused.
7. Make sure to dry brush the mattress and couches and air them in the sun to eliminate any bugs or insects, especially those that may not be visible to the naked eye.
