Hello doctor,

My wife woke up one morning with itchy bumps all over her chest and back. It has been four days since it started, and it seems to spread down her arms and neck. We need some advice. We have tried heavy doses of Benadryl and topical cream Cortisone, which was not helping. Please help.

Scabies is an intensely itchy skin infestation caused by the mite. Scabies can often be diagnosed clinically with a pruritic rash with the itching occurring predominantly at night and characteristic linear burrows over limbs; other family members can be affected. Regarding treatment, she can use a 5 % Permethrin cream single application below the neck all over the body during the night, leave it for 10 to 12 hours, and then take a bath. Use Permethrin medicated soap for bathing daily for seven days. Also, take the tablet Ivermectin 12 mg single dose same day. Wash your used clothes thoroughly and use fresh clothes after a bath. Next day apply Fluticasone 0.05 % cream twice daily for five to seven days and tablet Levocetirizine once daily for the same duration. Consult a specialist doctor, talk with them, and take medications with their consent. You review after one week.
