Hello doctor,

My inner thigh is having rashes which are similar to jock itch. I have been suffering from this condition for the past month. I have been applying Lulifin cream for three weeks twice daily. I have also been applying Terbicip cream 250 mg once daily for three weeks. Initially, it was reddish; now, the rashes have become black to light reddish in color.

I understand your concern and have reviewed the history and image provided (attachment removed to protect the patient's identity). It is tinea cruris. Tinea cruris is a fungal infection and is caused mainly due to stagnation of sweat and moisture in unexposed areas, including folds, private areas, and under abdominal and underarm areas. They also occur due to the infections spreading from an infected person or sharing belongings with the infected person due to lowered immunity and climate change. In addition, they may be aggravated by stress during harsh soaps washing the area, and so on.

The medications you have been using for the past month are great and helpful in reducing their intensity. However, they may take a few months due to the location and area of spread. In addition, they usually may spread and increase in size and may spread from one area to another if left untreated. Therefore, consistency with medication is key in having them treated. Also, as an end result, they may cause hyperpigmentation or dark patches after treatment which may eventually fade off with a continuation.

I suggest you to follow a few instructions mentioned below.

  1. Take a capsule of Itraconazole (antifungal) 200 mg once daily after dinner for four weeks.
  2. Use Ketoconazole soap (antifungal soap). It can be used to clean the area in the morning and at night. Lather with water and apply the lather to the affected area; keep for three minutes and wash it off. They may be continuously used till the rashes disappear. This medication is to be used on the affected areas only. On the unaffected area, a normal soap may be used.
  3. Apply Terbinafine (topical antifungal) cream 1 % on the affected area instead of Luliconazole (antifungal) cream. They may be used only in the morning and at night in the affected areas.
  4. Take a capsule of Becosule Z (multivitamin) once daily after breakfast for two months.
  5. Take a tablet of Cetirizine (antihistamine), 10 mg one hour before going to sleep. It can be taken for two weeks continuously or up to four weeks as you are already using it.
  6. Make sure to avoid sweating and stagnation of sweat and moisture. Make sure to wipe out moisture, if any.
  7. Use Terbinafine (antifungal) dusting powder on the affected area in the afternoon and the evening for absorption of moisture and sweat, and may be continued for months together.
  8. Make sure to avoid wearing tight clothes.
  9. Make sure to wipe the area well and make them dry before applying the creams each time after a bath.
  10. Lukewarm salt water wash may also be beneficial to avoid secondary bacterial infection and to avoid spreading of current lesions.
  11. Make sure to have a balanced diet with more fruits, vegetables, fiber, and proteins. And avoid oily and fried food, sweets, red meat, milk, and dairy products.
  12. Make sure to consume at least 13 glasses of water daily.

Consult a specialist, talk with them and take the medications with their consent.

Tinea cruris may take months to reduce in appearance, and the hyperpigmented patches will reduce in intensity with time and usually do not require any treatment.
