Hi doctor,

After 0.19 inch stone passed out of the urinary system, within few days, I have symptoms of frequent urination with burning sensation while urinating and a feeling of pressure in the lower right abdominal area. The symptoms started about three weeks ago. Medications tried are tablet Levofloxacin one week (suspecting UTI) and tablet Azithromycin and tablet Flagyl 10 days (suspecting urethritis) with no benefits. Urinalysis is normal. CT of the renal system is normal, the reports of which I have attached. Ultrasound seems normal, and also I did TRUS (rectal ultrasound). The doctor found nothing significant except calcification in the transition zone of the prostate, which he said is not that significant. Please guide me.

Thank you for consulting. A stone passage can cause inflammation in the lower ureter and vesicoureteric junction, which can cause symptoms like frequency and urgency. However, symptoms started three weeks ago and are not gone yet. So it is unlikely for me to be related to the stone passage. It is also unlikely for another stone in the system as CT is negative. I suggest you find the cause by doing cultures on urine and semen. Then, after the results, we can prescribe antibiotics that suit you. Urinary tract infection (UTI) is unlikely as you have taken tablet Levofloxacin, but I cannot rule out UTI without urine and semen culture.

Please follow up with reports so I can advice you further.

Thank you for the advice.

I did a urine culture before taking antibiotics for suspecting urethritis, and the results show no bacterial growth.

I was expecting this as, most of the time, the cause is prostatitis. And the most accurate result is expected with semen culture for prostatitis. Antibiotics treatment for prostatitis will be from four to six weeks; hence the result is essential before starting the antibiotics to select the proper one.
