Hello doctor,

I am a 45-year-old male. For the past seven to ten days, I am feeling occasionally uneasiness, an itching-type sensation in my left chest. I am not aware, of whether it is heart muscle or any other issue. I have no chest pain but have breathlessness sometimes. Also, I have no chest pain even during walking. Acidity occurs and feels the same in the chest area but not in the stomach. I am not a diabetic patient. My weight is 85 kgs and height is 167 cms. You suggested me for my hypertension issue thereafter I did not feel any major concern. Currently, taking Cilacar 5 gm after breakfast, Concor 2.5 gm after dinner, and Ecoasprin av 75 gm before sleeping. Kindly suggest fixing this health issue in line with my blood report and symptoms.

Most probably this is gastritis and reflux disease. Do you also have upper abdominal pain, nausea, bloating, burping, increase in pain on food, sour water feeling in the throat, or chest burning? These are the other symptoms that may be associated. However, it is better if you get ECG (echocardiogram) done, and if normal then TMT (treadmill test), a screening test as you are having risk factors like hypertension and dyslipidemia. Regarding hypertension, eat a low salt diet and monitor blood pressure regularly, and it should not be more than 140/90 mmHg most of the time. You should avoid a fatty, oily, and spicy diet. Do some walks after having food instead of taking a rest. Eat multiple small meals instead of heavy meals. Have regular sleep habits and avoid stress. Lots of green leafy vegetables and fruits increase the iron levels in the body. Avoid smoking and alcohol if any. Get a prescribed tablet of Pan DSR 40 mg or Esomeprazole and Domperidone combination before breakfast once a day for two weeks. Do regular exercises in order to some weight loss as well. It is better if you switch yourself to Tablet Razel (Rosuvastatin) A 75/ 10 mg instead of Ecosprin (Aspirin) AV due to elevated HsCRP (high-sensitivity C-reactive protein), so you may discuss this with your local doctor. There is a possibility fatty liver and weight loss will help in it as well. You should get vit D levels done due to elevated alkaline phosphatase. I hope I cleared all your doubts. Thank you.

Hello doctor,

Thank you for your reply. I do not have any symptoms of upper abdominal pain, nausea, bloating, burping, increase in pain on food, sour water feeling in throat, or chest burning. I do not consume alcohol or do smoking. Yes, anxiety and stress are there due to emotional instability in life. I have poor posture due to prolonged sitting at the computer for work. Maybe that poor postures (computer syndrome) added some issues to present heart health. Sure, I will follow your suggestion for exercise and diet and will get back after my ECG and TMT reports. As I am traveling for a week is there any serious issue to be attended immediately? Can I get my further test after a week? Should I start medicine tomorrow morning? Any other medicine would you like to suggest for ten days until ECG and TMT report? Any other test you would suggest?

Kindly upload the whole tracings (attachment removed to protect patient's identity). The machine-generated report which is mentioned there, may not be accurate always. However, from the last few complexes and reports mentioned on the ECG, this appears to be fine with minor changes like left axis deviation which is likely due to hypertension. QT (ventricular electrical systole) is fine in the complexes seen. It does not appear worrisome and though there are hypertensive changes, if possible please attach the picture of the actual ECG tracings. Thank you.

I just took pictures from my mobile camera for two ECG traces. I hope this will work. Should I continue my present hypertension medicine Cilacar 5 mg and Concor 2.5 mg, or would you like to suggest some more effective one in line with ECG and blood report?

Your ECG is fine, there are just some minor changes that are likely due to hypertension, and no need to worry about it. So get treadmill test done at a later date just to screen yourself. For hypertension, I do not think there is any need for a tablet Concor. It is a second-line drug nowadays, and we use it after other first-line medicines. Instead, I suggest you to take tablet Cilacar (Clinidipine) 10 mg once a day, and stop Concor. I hope I have cleared all your doubts. Thank you.
