Hi doctor,

My hemoglobin level is 5.1 g/dL. I get tired and exhausted often for a month. I also had hypertension for 12 years. Currently, I am taking tablet Telmikind H, half a tablet once a day. Kindly help.

I understand your concern. You are getting tired due to anemia, and it frequently presents with fatigue. Probably, you may have microcytic hypochromic anemia by iron deficiency. Your hemoglobin level is very low. I suggest you need an Iron sucrose-like injection for rapid improvement of iron deficiency. I would like to know if you have any history of bleeding? I also suggest you do stool analysis for occult blood for further work-up.

Thank you for the reply. My iron levels are normal, and I do not have any bleeding issues. I am taking iron tablets. Kindly help.

I understand your concern. Your iron level could be normal due to the iron tablet. There are four main causes of microcytic hypochromic anemia:

1) Iron deficiency anemia.

2) Anemia of chronic disease.

3) Sideroblastic anemia.

4) Thalassemia.

I suggest you discuss with your doctor for further management plan according to need. I also suggest you continue the iron tablet. An iron injection is needed when the hemoglobin level is less than 7 g/dL.
