Hi doctor,

I am 43 years old female. I have a marble size lump on the right tonsil with difficulties in swallowing and pain. I had a swollen tonsil for over eight weeks, and the lymph nodes present in the neck are slightly swollen. I was under treatment for thrush. Currently, I am taking Nystatin oral suspension. Please help me.

Please explain about the antibiotics given for you and how long you used it. Please say whether the lymph nodes in the neck are still present. Please tell whether you can palpate it. Please explain whether you had a fever and any bleeding problems. Please say whether a culture sensitivity of the throat swab is done or any other blood investigation is done.

I was given Phenoxymethylpenicillin 250 mg four times a day. The doctor also gave me Nystatin oral suspension. The doctor took a swab from my tongue, and it came positive for thrush. I took antibiotics for it. After that, my throat got sore, and I had a pain like a needle prick. I had pain on the right side of the chest when I swallowed. The lymph nodes were palpable. I had blood coming along with saliva. The doctor did an FBC before ten weeks, and they were normal. Also, a chest X-ray was done, and it was normal.

Take an appropriate dose of Ibuprofen or Tylenol for the pain and soreness. Take this medication thrice daily for three to five days. Take Pseudoephedrine or Phenylephrine decongestant for three days. Along with it, take Deslor 5 mg (Desloratadine) one tablet daily. If there is no improvement within three days, we have to go for another antibiotic. Take dilute warm water and mouth rinses. Avoid eating spicy and oily foods. Avoid smoking, alcohol and dust pollution. Drink plenty of warm water and other hot fluids. Take a picture of the throat and send to me once a while. Please say when you noticed the lump in the tonsil.

Try these steps and get back to me. If the tonsillar issue is due to stone formation, then it may not require gargles. However, a simple squeeze using a wooden tongue spatula will help to bring it out. But do not do it on your own as it is infected and it can cause severe pain.

I have a white spot, and it was a small dot. I noticed it before a couple of weeks, and it is present on the top of the lump skin. I disturbed it while trying to see tonsil. But, it has got bigger. What is it?

I have gone through the attachment (attachment removed to protect patient identity). It looks like a small thrush or ulcer. Please explain about the gel or ointment applied over it. Applying Tess (Triamcinolone) or Kenocort ointment is helpful. Please say whether you have pain over the spot.
