Hi doctor,

I had a lymph node enlargement under my left jaw five months back. I have been taking infection medicines, but two months ago, the pain reached my left breast on the axial point. Right now, there is inflammation and pain in the whole left breast, passing to the right axial point. Is this lymphadenitis? How do I know?

Your blood reports (attachments removed to protect the patient's identity) indicate a local infective pathology considering raised ESR (erythrocyte sedimentation rate) but no raised white blood cells. There is no change in hormone values, and breast tissue appears normal by USG (ultrasonography). I want to receive information about the diagnosis and treatment of jaw swelling.

Please let me know whether you are a lactating mother. Additionally, it would be a good idea to undertake either mammography or a CT (computed tomography) scan chest and abdomen at the earliest. Do you have a fever or sore throat at this time?

Thanks for the reply.

I am a 31-year-old unmarried woman with no children. There is no fever, but there is slight throat irritation. During the jaw or lymph node swelling, I went to multiple doctors who said it was sialadenitis. Then I went for homeopathic treatment, where the doctor treated me for lymphangitis, after which only the jaw swelling completely subsided. Attaching the list of medicines I had over time, the last one taken for breast swelling.

Could this be lymphadenitis?

As per reports (attachments removed to protect the patient's identity), it appears to be mastitis with reactive lymph node enlargement.

In the last scan, the mastitis had healed, and just a few enlarged lymph nodes remained without any major anomaly.

You have taken three rounds of empirical antibiotics with Augmentin, Clarithromycin, and Monocef, but strength and dosages are not mentioned. It would have been better if the treating doctor's prescriptions and records had been shared. However, since this is a second opinion, this is necessary.

You may or may not need another antibiotic regimen to clear the remaining infection. Also, in case of breast swelling in adult women, it would be ideal to get it checked by a local gynecologist, mammography, and a chest and abdomen CT scan if needed.
