Hello doctor,

I have been on Sertraline 100 mg, Amitriptyline 25 mg, and Chlordiazepoxide 10 mg for 15 years. They worked well. But nowadays, I am feeling depressed and unable to sleep with the above medications. I am a writer, and now I am finding difficulty focusing and lacking the right words to express. There is memory impairment also.

I started having stomach problems, acidity, flatulence, bloating, etc., for two years. I tried to switch to Fluoxetine 30 mg and also started taking Zolpidem for sleep. Please help.

I can understand your situation. You may have a major depressive disorder. There is also non-pharmacological management for your depression. You should try to follow the below guidelines to deal with it. It will help you.

1. Do exercises daily for 30 - 45 minutes, like at least going for a walk.

2. Do meditation for 20 minutes daily. Just sit at one place in a comfortable position and try to concentrate on your normal breathing. Do not stop or force yourself to stop your thoughts. Just let it be.

3. Eat healthy home-cooked foods, and avoid junk foods.

4. Drink enough water.

5. Spend quality time with friends and family daily. Or do video calls if you are living away from your family.

6. Spend time on creative hobbies like reading good books, writing journals, painting, drawing, learning new skills or language or musical instruments.

7. Take enough 7 - 8 hours of sleep daily.

8. Avoid the use of tobacco, alcohol, or any substance abuse.

Live your life, do not just spend it. Be bold and confident.

You should ask your psychiatrist to increase your medication. You can increase the dose of tablet Sertraline from 100 mg to 200 mg, and you can take the tablet Clonazepam 0.5 mg at night for sleep disturbance. You can add another antidepressant.

Increasing the dose of Sertraline cause severe stomach problem further. I contacted a psychiatrist where I am living presently. He prescribed a mood stabilizer, Valproic acid, which did not help me. I want to connect with a good Psychiatrist.

Yes, you can take Mirtaz (Mirtazapine). It will gain weight, not sugar. But still, if you have any doubt, go for tablet Bupropion. It is one of the best antidepressant medication.
