Hi doctor,

My brother is 25 years old and has had a fever for two weeks. Doctors confirmed that he is having a typhoid infection. We did report recently in the hospital, but he is becoming weak day by day with three to four hours of fever daily for the last seven to eight days. He is only drinking liquids for many days. We are worrying that he is becoming weak, please suggest any changes in medications and food so he becomes active.

Please note that the Widal test is noted as negative which means there is no typhoid (attachments removed to protect the patient's identity). They gave treatment for malaria mainly based on symptoms. Malaria medications cause gastritis and he may not be able to take food properly. I suggest the following:

  1. Tablet Pantop (Pantoprazole) 40 one tablet daily in the morning for 10 days.
  2. Tablet Ondiron DT (Ondansetron) 4 mg one tablet daily before breakfast for 10 days.
  3. Capsule Becelac Fortz (Nicotinamide) one capsule daily after dinner for 20 days.
  4. Eat small meals frequently- boiled rice, boiled potato, buttermilk, and some coconut water.
  5. Drink water in small quantities frequently.
  6. Repeat CBC (complete blood count) and CRP (C-reactive protein) test.

Thank you for the reply.

He got a fever today for two hours in the early morning, then he was given a tablet of Paracetamol and antibiotics. After which he started feeling better. He ate two Idly, a small cup of rice, and half a glass of pineapple juice. They did a Covid-19 test and came negative yesterday. Today they are planning for a culture test. If he is eating anything he is vomiting it out. He only wants to drink and not eat.

It is good that the doctors are giving correct medications and doing all tests needed. Give him small quantities of food, milk, fruit, etc frequently. Sip water frequently and make sure he urinates adequately.

Same symptoms don't mean you have the same problem. Consult a doctor now!
