Patient's Query

Hello doctor,

I am a 38-year-old male, and I have a mild fertility issue. I did a sperm test, and the doctor confirmed my fertility issue. We are trying to have a baby for several months. So kindly help me to solve my fertility issue soon.

Thanks.

Hello,

Welcome to icliniq.com.

I have seen your reports (attachments removed to protect the patient’s identity). Your sperm quality had deteriorated. The main concerns are abnormal sperm morphology and ASA (anti-sperm antibodies) titer. The intracytoplasmic sperm injection (ICSI) method is shown to help in such cases. Kindly abstain from alcohol, smoking, or any other substance use. Stress management and regular exercise are required. Take foods rich in antioxidants.

In such cases, high doses of CoQ 10 (coenzyme Q10) supplements (300 mg and above) help, but it takes around one sperm cycle of around two months to show effects.

Take care

Probable diagnosis

Teratozoospermia.

Patient's Query

Hello doctor,

Thanks for the answer.

Can CBD oil be responsible for this change? If my wife gets pregnant, is there a high possibility that my baby will have some genetic disorder?

Please confirm.

Thanks.

Answered by Dr. Aditya Gupta

Hello,

Welcome back to icliniq.com.

Yes CBD oil (Cannabidiol) could affect sperm quality but there are only anecdotal reports regarding it and the effect of CBD oil on sperm health is yet not confirmed. But it is advisable to avoid it. Abnormal sperm morphology do not increase the chances of genetic defects in baby. And most importantly try to manage stress.

Take care.

Patient's Query

Thank you doctor,

Thanks again for the doctor. I take CBD oil to fall asleep and I will stop taking it now. The doctor has prescribed me Zopiclone which I stopped taking it because I thought that it has serious effects than CBD oil. But now I think that I should start taking Zopiclone to fall asleep if my nightly meditation does not help me. But my question is can Zopiclone affect my fertility? I have been eating fruits with plain yogurt on a regular basis, do exercise daily, and seriously making changes in my lifestyle for a healthy pregnancy. Thanks.

Hi,

Welcome back to icliniq.com.

As I had earlier mentioned, the effects of sedatives (including CBD oil) on sperm health are not well established. Though Zopiclone is relatively safe in this respect, I would advise you to religiously follow the steps of sleep hygiene instead. You can take Zopiclone on an if basis. Follow sleep hygiene which is sleeping and waking up at a regular time. Do not take nap in the daytime. Avoid caffeinated drinks at least 8 hours before sleeping. Avoid using mobile, laptop, television at least one hour before sleeping. Have exposure to sunlight or white light after waking up. Also, practice relaxation techniques like breath relaxation exercise daily. Regarding medicine other than zopiclone you can go for zolpidem. Hope this helps.

Patient's Query

Thank you doctor,

Thanks, doctor. Because of the sleep problem and mild fertility issues, I am very worried about having a healthy baby. Please let me know whether sleep medications or insomnia can cause any abnormality in my future child. I want to make sure that my future baby is healthy and free from any health issues. I also have a six-year-old son. He is healthy and smart. But he was born prematurely at 32 weeks. Also, what leads to babies being born with a genetic disorder or other abnormalities? In my immediate family, there is no one with a genetic disorder. I look forward to your response.

Answered by Dr. Aditya Gupta

Hi,

Welcome back to icliniq.com.

There is no evidence that insomnia or sedative intake in fathers can lead to abnormalities in children. Insomnia on the other hand can affect general health which in turn can have an effect on sperm quality and can increase the chances of genetic damage. So it affects indirectly only. The risk factors associated with increased risk of genetic or birth abnormalities are the age of parents, history of genetic disorders in the family, a birth anomaly in the previous child, chemical or radiation exposure in pregnancy, use of certain drugs during pregnancy, and the health of parents. So the most important thing is to remain stress-free and adopt a healthy lifestyle in general.

Patient's Query

Hello doctor,

Thanks for your answers. I hope you still remember me. I want to let you know that for the last two months, I have been taking the following supplements regularly quite religiously:

1) 2000 mg of Vitamin C (1000 mg in the morning and 1000 mg at night).

2) 1 tablet of Vitamin D 25 mcg.

3) 3 capsules of Ashwagandha 300 mg (one in the day and two at bedtime).

4) Pro Fertil Capsule (multivitamins).

Please see the attached image (attachment removed to protect the patient's identity) with what each capsule contains.

Doctor, please tell me whether the supplements I am taking are sufficient to increase my chances for healthy sperm. Should I add or remove any supplement from the list? My wife currently has her ovulation window open, so our fertility doctor has suggested that we have intercourse every day. I want to know how many times in a day we should have intercourse (just once or more than that)? What is the ideal frequency to maximize our chances for a healthy pregnancy?

My wife does have PCOS, but it is under control. She is ovulating. I am 38, and she is 36. We have a healthy six-year-old son and want to have a second child. We are quite frustrated as we have been trying hard to have a second baby for almost a year, but it is not working out.

Would you please suggest what to do to maximize our chances? We are not yet ready for IVF treatment as it costs too much here in developed countries. If she does consider this treatment in my hometown, how long will she have to stay there?

I look forward to your response.

Answered by Dr. Aditya Gupta

Hello,

Welcome back to icliniq.com.

As I had earlier suggested, Coenzyme q10 has the most substantial evidence to help, and it is not there on your list yet. So add Coenzyme q10 300 mg in your supplements. There is no ideal frequency, and it is up to your personal choice. The main thing is to remain stress-free because excessive stress does affect fertility. So do not try too hard and be spontaneous and enjoy the moment.

I cannot comment on the duration required for IVF in your hometown as I do not deal with it.

I hope this helps and wish you the best.
