I have been married for a couple of months but unable to conceive a child. I have done a semen analysis test recently. I want to get the opinion of an andrologist over this report and discuss the treatment. I have got treated for varicocele ten years back.

Thank you for sending your lab result (attachment removed to protect patient identity). I can genuinely understand that you are worried because it is such a sensitive topic. I went through the results very carefully, and it does not look too bad. The morphology is fine, as is the total count and the general motility. However, the linear progression value is too low. What does that mean? It might be more difficult for you to make a woman pregnant, but it is not impossible. I do not know if this is your first sperm analysis. Therefore I recommend that you repeat it after three to six months. In the meantime, make sure that your scrotum is ventilated well and do not wear tight pants. You can try Zinc supplements. Take that according to the instructions of the pharmacist. The reason why I recommend this is that zinc slows down the degradation of free testosterone. So that would mean: Try to keep your scrotum as ventilated as possible, avoid bicycle rides, do not wear tight pants, and take zinc. Then repeat the test in a few months. I am quite sure that the treatment of your varicocele has nothing to do with the current situation.

Investigations to be done

Repeat test in three to six months.

Probable diagnosis

The probable diagnosis is Low linear progression value, and fertility most likely not significantly compromised.

Treatment plan

Your treatment plan is good ventilation of the scrotum, no bike rides, taking zinc and repeating the test.

Preventive measures

Follow as stated above.

Regarding follow up

Have a follow-up in three to six months, earlier if patient wishes so.
