Hi doctor,

I feel fatigue despite good sleep of about 7 to 8 hours. I have sporadic pain in the body. I feel cold and freeze rapidly especially on air conditioning. Otherwise, constant palm sweat is present. I am getting uncomfortable with lights. I have a sense of fullness in the head as if I am on a plane. I am getting fear of the future, especially of disease and death. I could not identify the source of these symptoms. If these are psychological, then what is the solution?

Welcome to icliniq.com. I want to assure you do not worry as everything is going to be fine if proper care and treatment is opted in for. I have thoroughly gone through your case and can well understand your genuine health concerns. Such an issue happens due to either of the following causes. Conditions like thyroid disease, diabetes mellitus, hepatic disease, heart disease as endocarditis, hypertension and autoimmune disease where body's immune system gets against the body's own tissues, may also have a psychological part to the disease. Can you please tell me how much time elapsed for such symptoms? Is this a continuous one or on and off? Do you or anyone else at your home know that you snore while sleeping? What are your blood pressure, pulse rate, and respiratory rate today? Respiratory rate is when you sit quiet and count for one minute that how many times you inhale and exhale, a complete respiratory cycle. Pulse rate, you may count by keeping the finger on wrist and counting the feeling of pulse in one minute. Do you have fever? How active are you? Can you please upload the tests you went for? Diagnosis is the key to proper treatment. We can rule out certain causes from diagnostic tests. If not available, no problem you must get CBC (complete blood count) with ESR (erythrocyte sedimentation rate) and differentials, fasting blood sugar levels, fasting lipid profile, thyroid profile, EKG (electrocardiogram), total CPK (creatine phosphokinase) levels, HBSAg (hepatitis B surface antigen) and anti-HCV (anti-hepatitis C virus) levels tests done as soon as possible so that we may proceed further. Treatment will be decided on follow up details. Do not worry, it can be managed.

I feel these symptoms since two years from the time when I found to have high blood pressure. These symptoms come and go. I will not snore at night. My BP varies with an average of 137/88 mmHg and to the maximum of 144/94 mmHg. My pulse rate is 70 to 90 bmp. There is no associated fever. I was an active man but not now. I did all the tests except HBcAg and anti-HCV. All are within range and in addition I did echo and sonar for liver, stomach, kidney and pancreas along with function tests. Everything was fine.

Welcome back to icliniq.com. At this age group, such a high blood pressure is not good. You need proper medications for blood pressure and you need to take them lifelong. In some cases, I have seen my patients getting off of medications for high blood pressure by their diet and exercise. Your Body Mass Index (BMI) is high. If you reduce 10 lbs of weight, it will benefit you in a drop of almost 5 mmHg blood pressure. Your BMI should not be more than 24.99. Right now your BMI is at 26.42. High blood pressure makes the blood vessels stiff and organs are not properly provided with blood and the required nutrients. This causes muscles to ache here and there, unexplained fatigue and it also weakens the heart, kidneys and lungs. If left as such for longer, it may pose serious risk to your heart and kidneys. Weak heart is unable to pump blood properly to profuse the organs and that leads to the issues like you are facing. Please do the tests for HBSAg, anti-HCV, liver function tests, and kidney function tests. We need to rule out if you have any liver disease like viral hepatitis. Also we need to check your kidneys to see how they working. If you have anyone of these tests recently done, please upload those reports for review. If you are sexually active you also need a test for HIV. It is mandatory to test for HIV at this age at least once. Vitamin B12 levels and Vitamin D levels also need to be tested. If you spend more time indoors, it leads to low vitamin D levels. Try spending some time outside. Do not forget sunglasses and apply U-Veil SPF-60 sunscreen lotion while going outside in sunlight. A probable diagnosis could be hypertension. It is recommended to take a multivitamin which includes Vitamin B12 and Zinc in it, once daily for 4 weeks. Engage in activities like jogging, swimming, or cardio exercises as it will help you get better soon. Do not worry as things are under control at this stage. But be careful for the future. Try to complete a target of 10,000 steps of jogging at a healthy pace daily. Your smartphone may have a pedometer in it, or you may take a separate one of some good quality. Avoid red meat, spices, extra salts and sugars. Take plenty of fruits, vegetables, salads and keep a judicious supply of clean drinking water. Keep a check on your blood pressure. Make a chart of your blood pressure and pulse rate readings. Measure twice daily and make a chart with date and time. After few days upload a picture of that paper in a follow up query.
