Hi doctor,

I am a 30-year-old doctor. I have had a black papule on my forehead for 1.5 years and that is associated with itching. It got subsided four to six months after using Mometasone and multivitamins but has reappeared then. It first appeared just below the hairline and spread to contiguous areas below and above the hairline with loss of hair in the affected area. The lesion is black, hard, and itches frequently. There is no scaling, bleeding, or any pus discharge associated with it. Please observe the sun tan surrounding the lesion in the images attached. Thank you.

I have reviewed the attached photographs (attachments removed to protect patient identity). You are having lichen planus. It is a chronic disease of the skin due to immune dysfunction. I would recommend you to apply Mometasone furoate cream on the affected areas twice daily along with tablet of Hydroxychloroquine 200 mg twice daily. This needs to be taken under the direct supervision of the doctor after getting your liver function tests and fundus examination. It will take time to resolve and the hair loss is permanent. For itching you can take the tablet Levocetirizine 5 mg once or twice daily depending on the intensity of the itching.

    Same symptoms don't mean you have the same problem. Consult a doctor now!
