Hi doctor,

I gave birth to a boy 11 months ago. I stopped breastfeeding him six months ago. Then I started menstruating and it was regular. But since last month, I am bleeding weirdly.

I understand your concern.

After delivery, menstruation could be irregular and unscheduled. Pregnancy then breastfeeding affects menstrual regulation and the body takes time to be normal and get back to the previous menstruation schedule. In this period, you can get pregnant easily so make sure it is not post-pregnancy bleeding. To check this you have to do serum beta human chorionic gonadotropin (hCG) immediately it will give you the exact status of your pregnancy. If it comes positive do an immediate urgent transvaginal scan to know if you need emergency evacuation or not as with retained products you can have too long bleeding. If beta human chorionic gonadotropin (hCG) comes negative then you can have tablet Ponstan (Mefenamic acid) 500 mg eight hourly and tablet Tranexamic acid 500 mg eight hourly for three to five days as per symptoms. When bleeding reduces or stops with no pregnancy you can have combined oral contraceptive pills for three months to regulate cycles, to prevent unscheduled bleeding and it will act as a contraceptive as well. Eat a healthy diet with lots of iron containing goods like apples, carrots, spinach, and iron supplements as well. If after three months of oral contraceptive course if you still experience irregular periods then take transvaginal scan, thyroid function test, and complete blood count. These laboratory results are to be discussed with gynecologist here or locally.
