Hi doctor,

Two days after my partner's last period we had sex. I am sure that the sperm did not go inside. According to the last period schedule and as per 28 days cycle, she was expecting her periods by yesterday or today which has still not come. I have some queries. What are the chances of her being pregnant (which should not be the case because we had done sex in a similar way previously also without any issue)? Can she still expect her periods considering a slight delay? What are the methods or characteristics so that she can diagnose if there is any pregnancy? If yes, then considering this is still an early stage (around 28 days) what can we do to avoid pregnancy?

I do not see any chance of pregnancy. For any lady, ovulation will be around 12-16th day of the period. Life of egg is 24 hours and that of sperm is 72 hours. As you had sex on day 7 of periods, there is no chance of sperm being alive till date. Ovulation and the egg will not be available on the 7th day of periods. So in total, there is no chance for pregnancy, delay in periods is not due to pregnancy. It can be delayed even if any significant mental or physical stress, any severe anxiety, or depression.

As you had sex so early in periods when there was no egg available and with full protection, there is no chance for pregnancy. Chances of pregnancy are zero. You can wait for periods up to 7-10 days after the expected date, this is normal. If you want to diagnose pregnancy, it can be done with a home urine pregnancy test, earliest one day after the expected date. If doubtful, it can be confirmed with blood serum Beta hCG (human chorionic gonadotropin) tests. This can diagnose the smallest of pregnancy anywhere in the body. There is no chance of pregnancy, but if you still think there is a chance then you will have to have MTP pills (Mifepristone) once pregnancy is confirmed.

Just to let you know we had sex without protection but I did not let the sperm go in by withdrawing just before ejaculation (and this way I have done previously also without any pregnancy). So there might be a slight chance of very little quantity of sperm went in, however, still in the absence of eggs very rare chances of pregnancy. Please provide your input on this point as well. Now consider for a moment that she still might be pregnant, considering the first month how should she feel? What changes she will feel in her body and how we can identify them? Can you please suggest some pills as well if this is the case? Names and side effect, effectiveness of the pills, etc? I am asking you all these things because the situation is a bit tricky for us since we are getting married next month and would like to hide a few of the things for obvious reasons. As she has not yet felt anything yet on pregnancy, how soon she can expect her due periods? What is the normal delay duration?

Withdrawal method has the highest failure rate. Single sperm is enough for pregnancy. The clear fluid called preseminal fluid or precum is the clear fluid that comes out before semen, it is rich in young and healthy sperms, so the chances of pregnancy are higher. You did not get pregnant as may the sexual exposure was not around the ovulation. But considering the day of periods, there is absolutely no chance of pregnancy. There are no fixed symptoms for pregnancy. Usually, the features are vomiting, nausea, weakness, headache, so on. But the symptoms keep changing even in the same person in subsequent pregnancies.

Worst comes if still pregnant, pregnancy has to be confirmed as inside uterus, this can be done only at two weeks of urine pregnancy test showing pregnancy. Pregnancy inside uterus is confirmed with scan, as if it is in the tube it can bleed inside and be risky to the mother. Then she will need a prescription from gynecologist, as this medicine is available only with the prescription. Just wait for 7-10 days, she will get the periods. You are unnecessarily getting tensed. Just relax, she will get periods.
