Patient's Query

Hello doctor,

I suffer from cracking neck and feeling nerve stinging or nerve compression from time to time. As I am close to healing, I injure myself. It has been going like this for about six months. I could not walk but could walk again without pain after taking the NSAID treatment. About a couple of weeks ago, I came back to visit my family. The weather was very cold. I felt stiffness in my back and felt like I had touched my nerve again. I had weakness in my hand, but I regained power lately. Now I feel some tingling sensation like pins and needles in my pinky finger and the finger right next to it. I want to know if I am healing or if it is a bad thing. Should I visit the doctor again? Kindly suggest.

Hello,

Welcome to icliniq.com.

As per your description, I can conclude that you intermittently have neck pain or stiffness, radiating pain in your arm and hand with tingling or pins, and needle sensation in your little and ring finger. Has it been diagnosed till now or not? I would like to know your symptoms in detail.

  1. What are your symptoms exactly? Please describe in detail.?
  2. These symptoms get exaggerated in which situations?
  3. Have you undergone any investigations apart from an X-ray and cervical spine?
  4. Are you taking any other medications apart from Voltaren SR?
  5. Has any particular diagnosis been made for your symptoms till now?
  6. Do you have any other medical history, or are you on regular medications apart from these neck symptoms?

As per your description, there may be a possibility of cervical spondylosis with radicular pain in the arm. If you have neck stiffness, pins and needle sensation, and tingling in your fingers, you should visit the doctor. And as you mentioned, such symptoms happening intermittently should be evaluated in detail.

Thank you.

Investigations to be done

MRI (magnetic resonance imaging) cervical spine nerve conduction, study bilateral upper limbs with mid palm study. Serum vitamin B12, serum vitamin D3, serum calcium, or phosphate.

Regarding follow up

Revert with the above information as mentioned above details for better understanding and further judgment.

Patient's Query

Hello doctor,

I will answer all your questions one by one. All the symptoms below are just on the right side.

  1. My symptoms are stiffness below my neck around C-8 to T-11. I think sometimes I feel like a nerve is pinched, and when I was injured, I felt pain when I took a breath, but it is gone now. I only felt a scorching pain under my trapezius muscles when I walked at night. It was very cold, and my muscles were stiff. That is when I started the treatment again. Before that, almost all my symptoms were gone except for my hand and elbow, but I think it is because of my sleeping position because I only sleep on one side. Also, the hand symptoms come and go. Right now, they are not that bad. Sometimes I feel discomfort in my wrist also. All these symptoms are only on my right side, but after taking Voltaren, the symptoms went down slightly.
  2. Symptoms get exaggerated with cold weather when I sit scrunched working on my computer, if I stay in one position for a long period, or if I sleep on my right side.
  3. No, I only did the cervical X-ray.
  4. I am taking the tablet Voltaren, which is anti-inflammatory.
  5. No, my doctor did not explain anything to me. He asked me to take the medication and start training, but it aggravated when I started doing pushups. I can only run as running makes me feel good, and taking a hot shower.
  6. I had a reflux kidney operation when I was 5 years old. But it is good now.

Hello,

Welcome back to icliniq.com.

As possible, it cannot be cervical spondylosis with radicular pain in the right arm. It may be associated with muscle spasms in the neck.

Thank you.

Investigations to be done

MRI (magnetic resonance imaging) cervical spine nerve conduction. Study bilateral upper limbs, serum vitamin B12, and serum vitamin D3.

Differential diagnosis

Cervical spondylosis with radicular pain.

Treatment plan

Can try the medication Gabapentin or Pregabalin and Tolperisone after consulting your doctor along with physiotherapy of the neck.

Regarding follow up

Review as and when required.

Same symptoms don't mean you have the same problem. Consult a doctor now!
