Dhat and nocturnal emissions (nightfall) are common psychosexual issues among Indian males. Dhat syndrome is a neurotic syndrome characterized by vague somatic symptoms such as fatigue, weakness, lethargy, body aches, reduced energy levels, anxiety symptoms, and sexual weakness; all these symptoms are attributed to semen loss in the form of excessive masturbation, nightfall or along urination. The person becomes too preoccupied with semen loss that it becomes a vicious circle, increasing the anxiety and further aggravating the problem.

The most common cause of Dhat syndrome is the lack of sex education, adequate information about sex physiology, and wrong concepts about semen formation and discharge. Hence patient misattributes his physical symptoms to his sexual activities.

Comorbid conditions such as erectile dysfunction, premature ejaculation, hypochondriasis, somatization, depression, and anxiety must also be managed.

Management of Dhat syndrome involves:-

1. Detailed physical evaluation and manual examination of genitals.

2. Sex education about the anatomy and physiology of sex organs, masturbation, semen formation, and nocturnal emissions.

3. Relaxation exercises in the form of JPMR (Jacobson's progressive muscle relaxation technique) to reduce somatic symptoms of anxiety.

4. Anti-depressants or anti-anxiety drugs, if needed, can also be started for a while.

Dhat is manageable and can be cured.
