Q. What could be the reason of a persistent and a non-itchy rash?

Hello doctor,

I am a 25-year-old male, and I fingered someone with a superficial one-day-old cut on the finger. My complete STD and Hepatitis panel test at 52 days was negative. At 42 days, the rapid immunochromatography test for HIV 1 and 2 was negative. At 49 days, CMIA HIV 1 and 2 antibody screening test was negative. At 53 and 57 days, CMIA antibody-antigen p24 HIV 1 and 2 CMIA test was negative too. Minor non-itchy rashes on my upper arm were seen two weeks after the exposure and are present even after eight weeks. Little fatigue is still there. Are the test results conclusive, or further testing for HIV is required? Kindly suggest.

You have gone through all the tests, which were negative, which means you do not require further tests. Your complaint of non-itching rashes that appeared two weeks after exposure and persisted till the eighth week can be due to allergy to any subject that you used for the first time, typhoid, infestation of the worm, fungal mycosis, and lastly, HIV (human immunodeficiency virus) infection.

For the rash, I would suggest -

  1. Tablet Azithromycin 500 mg, once daily for six days.
  2. Tablet Fluconazole 150 mg, once daily for five days.
  3. Tablet Stercort (Deflazacort) one after lunch and one after dinner daily for six days.
  4. Locally apply ointment Fucidin + Cotrimoxazole + Betnovate -CIN 1:1:1 ratio two times a day after cleaning up with any disinfectant.

Advice -

  1. Do not use an injured finger.
  2. Regularity in food habits.
  3. Avoid consuming restaurant food and eat homemade food.
  4. Add citrus fruits like lemon, orange, kiwi, gooseberry, etc.
  5. Add fresh curd to lunch.
  6. Include sprouts in your food as they are an excellent source of antioxidants.

Causes for fatigue can be -

  1. An imbalance between rest and work.
  2. Irregular habits.
  3. Stress.

Treatment -

  1. Capsule Revital (vitamin A, C, D E, and K) for males one at night.
  2. Capsule Cod liver oil once daily can be used for a long time for better results.
