Patient's Query

Hello doctor,

I had unprotected oral sex for one minute 11 weeks ago. After two days, I noticed frequent urination, and after 12 days, slight pain or burning when urinating. I made three tests, two for chlamydia and gonorrhea and one for all microbes and HSV-1 and HSV-2, and HPV. All came back negative. I have been on several antibiotics four times for ten days. I am also taking Valtrex 500 per day. My main concern is still HSV, although it was negative ten days ago and before I took my first Valtrex pill.

1. What is your opinion of the chances of having HSV with no visible symptoms and a negative test?

2. It was a PCR urine test, but do I understand correctly that it should be reliable, especially if lesions are in the urethra?

3. If it is not HSV, what can it be?

4. What should I do now?

Herpes molecular test detects the HSV (herpes simplex virus) RNA load and is the gold standard test in diagnosing herpes. You do not have a blistering sore on the genital which is usually present in herpes infection of the genital. So here, the possibility of herpes is significantly lower. Suppose you are having a problem with burning micturition. In that case, a routine microscopic examination and urine culture report can be done to find any causative agent if present and treated accordingly. Investigations have already ruled out other STDs (sexually transmitted diseases) like chlamydia and gonorrhea, as per history. So investigate with a urine culture. If the cause is unclear even after the urine culture and a frequent burning sensation is present while urinating, then a urodynamic study and a pelvic scan can be done. Consult a urologist for examination and discuss all these.
