Hello doctor,

I am a 29-year-old male. I got an unprotected blowjob from a prostitute at a massage parlor three days back but I do not know her HIV (Human Immunodeficiency Virus) status. I only had oral sex once and I did not ejaculate in her mouth. She then cleaned all with a towel and I washed my penis with soap and water. Later I got HIV-1 and HIV-2 combination with a p24 antigen test which came negative. Please help. Should I go for PEP(Post-exposure Prophylaxis)? I am not able to sleep what should I do?

The chances of transmission of HIV (human immunodeficiency virus) by single unprotected oral sex is almost nil unless there are bleeding wounds in the oral cavity. If the person is HIV-negative then there is no chance. The HIV antibodies negative test results suggest negative status as of now but are not conclusive for this episode. PEP (post-exposure prophylaxis) may not be needed if the act is limited only to this. Please consult your doctor for further risk assessment and the need for PEP. PEP if needed has to be initiated as early as possible not later than 72 hours of exposure.
