Hi doctor,

I had two encounters with commercial sex workers two months back; I used protection for penetrative sex with both of them, and with one of them, I did not use protection for oral sex for a very short time. To the best of my knowledge, the condom did not break while having penetrative sex. I am worried, I might have contracted HIV. I double-checked with them about their HIV status; they said they were negative. Before this few months back, I had sex with a commercial sex worker once, and I got myself tested for HIV RNA-PCR, a month after the encounter, the test results were negative. Should I consider that test conclusive? Or should I go for another round of tests?

1)Oral sex carries almost a negligible risk of transmission. The risk of transmission, even if we assume her to be positive, is zero. If she was an active partner during oral sex, saliva could be hostile to the survival of the virus.

2) If the tests came negative after a month and there are no symptoms, you do not have to go for any tests.

I have a few more concerns that I need your advise about. I am getting mild rashes on the thighs and have mild joint pain on my left arm, loose stools with no fever, or body pain. Is this relatable to HIV? And should I take tests to be assured of even the slight chances that I might have acquired HIV? Is it because I used condoms that I do not have to worry about the latest encounters? Could you please look at the report I have attached, help me with my concern, and explain what it means by an active partner during oral sex?

1)These symptoms are non-specific and are not related to HIV.

2)The latest encounters do not require tests as they were protected.

3)As the RNA-PCR test was negative after your last unprotected encounter, you do not have to worry.

4)Active during oral sex means the partner was licking your penis. This reduces the risk because there is no risk of transmission unless there is an active bleeding lesion in her mouth. If you had licked her vagina, then there might be a slight increase in risk, but even then, the chance of getting HIV would have been almost nil.

I had a look at the image attached (attachment removed to protect the patient's privacy), there are various reasons for this condition.

1)This can be seen in HIV but is not a diagnostic for HIV. Most of the time, the reason is idiopathic (spontaneously without any cause). Observe for any factors and consult a skin specialist.

2)As I told you, your tests are negative, and the sexual activity you had was a minimal risk activity. It is not possible that you have contracted HIV. If you are still anxious, you can get an HIV duo (antigen and antibody) test; it is only to make you feel assured. I can guarantee that it will come negative.

1) I have gone through your reports. HIV and HBV (hepatitis B) are negative and non-reactive so, you do not have to worry about that.

2)One thing to be concerned about is that your IgM antibody (antibody released as a response to specific infections) for the herpes virus is positive and means you have a herpes infection that should be appropriately treated. I suggest you take tablet Acyclovir for a week.

Otherwise, remain relaxed as all the other tests are negative, and the blackening of nails is not related to any STD (sexually transmitted diseases).

Same symptoms don't mean you have the same problem. Consult a doctor now!
