Hi doctor,

I just delivered a baby five months ago. Since two months, I have this recurrent blood clot in my mouth (photo attached). Whenever I wake up, I feel it, and it comes up as a huge blood clot. Initially, it is not painful but is at the same site each time. And later, it turns into an ulcer that was very painful. This is the third time in the last two months. I am getting a tingling feeling before the blood clot appears. Could it be due to any deficiency or could I be biting it in my sleep?

The bruise on your lower lip looks like bleeding from an ulcer (attachment removed to protect patient identity). As you have mentioned that it is recurrent in the last two months, I suspect it to be the normal oral ulcers. Please do not attempt to pierce it. Keep observing for any color change. If yes, then get it checked with biopsy and that could be a cyst. If not, then it can resolve automatically with time. Also, as you doubt, it could be due to stress, vitamin deficiencies, etc. For pain relief, I suggest application of 0.2% Lignocaine gel with clean fingers and leave it for 5-7 minutes and then wash your mouth with normal water. Consult your doctor, discuss with him or her and take treatment with consent.
