Hello doctor,

Today I broke my bracket. It is one of my top right back teeth. I am about eight months into my treatment. I have a minor case, and everything is now assumably straight (just some unevenness). The bracket that broke does not have an elastic tie on it (same as the brackets in the same place on the top left side keep in mind all my other brackets ( but the ones I mentioned, which are 2, 15, 18, and 31). I will be able to get an appointment once I get back, which is next week. Is my bracket self-ligating? Also, do you think my teeth will shift? And if so, how severely, and will it be only the tooth, or will it affect the area (keep in mind this tooth was pretty straight before)? And when it gets fixed (which is sometime around next week), will it take a while for the tooth to shift back, or the teeth around it give if it affects the others?

I understand your concern. Breakage of the bracket during orthodontic treatment is not an unusual occurrence. It sometimes happens due to multiple factors involving patient and doctor-related factors. Towards the back side of the teeth, molar tubes hold the wires and are placed on all four sides (the right and left sides of both the jaws). The tubes are hollow and open-ended on both sides to let the wire pass through them. Hence, no elastic tie is required on the tubes. No, your brackets are not self-ligating but a conventional ones only. Self-ligating brackets have a sliding door on the front aspect of the bracket that acts as the fourth wall of the bracket, while yours are front open brackets. A molar is a large tooth that requires more time and force to move it. So, there will not be any major changes in the tooth position. It would remain the same only, with minor changes that might occur. So, this is not a severe condition, as you will see your orthodontist soon. The changes that might occur (low chances of occurring, though) will affect only the tooth without a bracket and not the whole area. All the other teeth are under the influence of brackets and wires, so they will not experience any unwanted changes. It may take up to one appointment duration to get things back to the stage where it was before breakage. This happens because, on re-attaching the molar tube, we are only sometimes 100 % accurate in positioning it to the same angle and positions where it was just before breakage. That is why we step down the wire (go to a smaller dimension wire than the current one) for an appointment to get the alignment again, followed by stepping up again in the next appointment. Mostly, there is no effect of stepping down of wire to all the other teeth. They remain the same the way they are in the current wire stage. Hope this clarifies your concerns currently.
