Hi doctor,

I have severe pressure in my inner ear. I went to the doctor last week, and he cleaned my ear. But now, the pressure is intense, and I cannot hear anything. I have antibiotic drops, but they do not seem to help. I had this problem about ten days ago, but the pressure is getting worse. I was diagnosed with COVID four weeks ago, and no symptoms have improved. I am currently taking Cymbalta, Propranolol, and Lovastatin. Kindly suggest me a solution.

I went through your query and understood your concern. I assume pressure means you are not hearing anything in that ear and it feels blocked. If this is the case, it should be serous otitis media. In this condition, there would be a collection of fluid behind your ear drum. So the ear feels full. Therefore, I suggest you to follow the instructions mentioned below: 1. Do Valsalva exercises 15 to 20 times daily. 2. Use Xylometazoline (nasal decongestant) nasal drops. Use three drops three times daily. 3. Do steam inhalation three times a day. 4. Avoid foods like bananas, citrus fruits, ice cream, and yogurt as they might cause allergies. Following these remedies should improve your condition in 10 to 15 days. Unfortunately, this usually happens because of eustachian tube dysfunction. If you have pain and discharge in the ear, kindly get back to me as the diagnosis changes. I have assumed pressure in the ear since you have not mentioned anything about pain or ear discharge. I hope the doctor who cleaned your ear just removed wax, right? Did he say whether the pressure is due to discharge or ear inflammation?

Thank you for your reply. No, he just cleared the wax. He did not look at the inner ear, but he checked to make sure his nurse had removed the wax completely. I do not have pain or discharge in the ear. I feel a lot of pressure inside the ear. Please help.

In that case, I suppose that my diagnosis is correct. Therefore, do not worry, and I suggest you to follow the instructions mentioned below: 1. Levocetirizine with Phenylephrine (antiallergic medication) one tablet at night for 15 days. 2. Xylometazoline (nasal decongestant) nasal drops three drops, three times daily. 3. Do steam inhalation three times a day. If your condition does not improve after two to three weeks, I suggest you to consult with an ENT (ear, nose, and throat) surgeon. They will have to do an otoscopy and look into your ears and see the status of your ear drum. If fluids are found, they will advise for an impedance audiogram and then do a minor surgical procedure to introduce ventilation tubes into the drum, which will drain out the fluid.
