I have PCOS. Currently, I am on tablet Metformin and Letrozole. Still, I am not ovulating. My LH levels are 13 IU/L, and my FSH is 4.9 IU/L. Please help.

I can understand that you are worked up about not ovulating despite being on multiple ovulation induction medicines. Please answer the following questions.
1. Have you got an ultrasound done?
2. Did the ultrasound show that the follicle is growing?
3. Since when are you taking these medicines?

My follicles grow up to 25 mm but are not rupturing. Also, my endometrial thickness is 4 mm on the 20th day, and my LH levels are high. What should I do now?

Have you been given HUCOG (human chorionic gonadotropin) injections when your follicle reaches the appropriate size?
1. Your high LH levels are suggestive that your PCOS (polycystic ovarian syndrome) is not under control. You could stop the ovulation induction for three months and try oral contraceptive pills to reduce the LH levels.
2. You could use ovulation induction with injections instead of tablets. This depends on how many cycles you have used the tablets for.
I think you should try oral contraceptive pills and then restart ovulation induction. However, please consult a physician, discuss with them and start taking the medications with their consent.
