Hello doctor,

I got a groin injury six months back, and I have pain in the right groin area, particularly above the iliac crest, referred to towards the right hand. I got an MRI (Magnetic Resonance Imaging) done, which shows edema in the iliopsoas muscle. No avascular necrosis is seen in the femoral head. The femoral head is normal in size. No significant joint effusion is noted. Got a subsequent MRI (Magnetic Resonance Imaging) last month, and swelling has been reduced, and no obvious fluid collection is observed. I still have pain in my right groin above the iliac crest. I am enclosing my MRI reports. Kindly suggest.

According to the reports (attachment removed to protect the patient's identity) attached, you have a right iliac transplanted kidney, and there can be a temporary relationship between the groin pain and the transplantation. What was the mechanism of injury? Also, it would have been more helpful if you had sent the files rather than the report, as it does not give me any information about other structures. The edema is reduced in the subsequent MRI (magnetic resonance imaging), but we cannot be sure this was brought about by the injury. You have sent a list of medications, but these are not the names of the drugs but rather company names. If any of them are anti-inflammatory medications, then take them after considering you have a kidney transplant. I hope this helps you.

Hello doctor,

I have been walking briskly, but in the month of the injury, I got stretched in the right groin near the sacroiliac joint. But I continued my walk, assuming the pain would go away. My nephrologist did not understand the intensity and told me to continue my walk. But one day, pain spread across the groin, including the muscle above the iliac crest, so I got an x-ray done but found no bone abnormalities in the hips. The pain started increasing, and I was referred to an orthopedic surgeon. Based on his advice, I did an MRI three months ago. Since I was not allowed to take anti-inflammatory drugs, I was advised to rest and see if there was any infection. I did not have a fever, and gradually the pain subsided partially. Then I got my second MRI done last month. The doctor told me the report showed insignificant edema and that there was no need to worry as it would recover. He asked me to start daily activities slowly and do pranayam. When you said file, do you mean MRI scan CD? Medicines prescribed to me are immunosuppressants, vitamin D, and antidepressants. None of these are companies' names. Since the pain persists, I thought of taking your experienced opinion to know if I am on the right track or if your advice is a better approach to resolving the issue. Kindly advise, particularly for patients having a kidney transplant.

I am aware of the kidney transplant. The medications you have written are the names of the company and not the actual drug name. Secondly, I meant to see the MRI pictures, not the report. Thirdly try the tablet Paracetamol or Tramadol, as there are safe painkillers for a kidney transplant. Finally, to be sure, you need to be examined physically as the symptoms you mentioned do not explain the iliac muscle inflammation, and there is no obvious cause unless you mention the timing of the transplantation. It is possible that a nerve or vessel may have been damaged to cause the pain. Try walking again to see if the pain resumes.
