Hello doctor,

I am in my mid 20’s and I usually recover from most things quickly, but this has persisted for some time. I also quit smoking since in the beginning whenever I would smoke, it would feel like I am burning inside my private area. My health issues started after waking up one morning to groin pain and slight pain in my right testicle. A few days later I masturbated and during ejaculation, it felt like I pulled a muscle in my right testicle. From then, my issues persisted. I had constant inflammation on the right side of my penis, which led to the frequent urge to pee since it seemed like there was actually swelling on the right side of my penis. I had urine tests run and there were no infections or STDs. I was on Cipro for two weeks, which did nothing since I guess it wasn’t an infection. After finishing the course of antibiotics, I masturbated again, to see if the problem persisted. Immediately after this, my symptoms got worse. Everything had a reoccurrence. At one point it got so serious, I went to the ER because I developed severe testicular pain. They did an MRI on my testicles and ran some urine tests again, and found nothing. I was sent home without even a prescription. From then, I decided to go the physical therapy route. I did cardio on the treadmill for two hours straight, and my urge to pee went away immediately. The pain in my penis also started to decrease. After the gym each day, I see noticeable improvements.

My main concern is yesterday, after physical exercise for ten days, I wanted to, “test” myself again, and see what would happen after sexual activity. Sure enough, after ejaculation, the inflammatory pain and urge to urinate started all over again. It stopped after I exercised for two hours; The pain has subsided and the urge to pee is not there any longer. Fortunately, I do not have burning when I pee or pain in other parts of my body. Did I pull a muscle in my groin area and it needs weeks to recover? If anyone has seen cases like this, how long should I avoid sexual activity? Is this condition rare or something that may need further attention?

From what I have read, it seems like you have some degree of prostatitis and urethritis. Please help me with a few details so that we can get to the bottom of this issue: Do you have diabetes, hypertension, or any other illnesses? Are you sexually active or masturbating regularly? It would help if you could specify in times per week. Have you ever had any surgeries done in the past? Do you have trouble peeing like straining to pee, or a feeling of incomplete bladder emptying? Do you have any pain in the perineum, the area between your testicles and anus? Do you have a burning or stinging sensation while peeing or have any discharge from the urethra? Did you have unprotected sexual contact over the last month or two? Do you sit for prolonged hours in one go? Lastly, I wish to know where you are located, because that will dictate the tests and medications I can order. Is it possible for you to upload your scan reports (urine, blood, and MRI scan reports)?

Hello doctor,

Thank you for replying. I am not diabetic or hypertensive. I had my blood pressure checked two days ago before getting my Vitamin C IV at a medical spa, and the results came back perfect. I have not been sexually active in the past (six) months. Previously, I would masturbate maybe twice weekly. I have never had any surgery in my life. I do not have pain during peeing or feel my bladder is not emptying fully. It is just after the pain, a few minutes later, the pain in my penis starts, and I get relief from peeing, so my mind tells me to go pee. Physically it is hard to resist the urge since my mind keeps telling me to do this. My lifestyle is pretty calm. I do have the luxury of relaxing for long periods of time, for the past year. This can include sitting or sleeping during the day. If you can recommend any medication, kindly suggest it.

It seems like you have a degree of prostatitis (inflammation of your prostate). I suggest you some tests and medications. You may need a prescription from your doctor to get yourself the medication. I will also give you some lifestyle advice. Tests to do before starting the meditation- Urine culture and sensitivity test, and semen culture and sensitivity test. Start the medication immediately after giving the tests. General measures to be taken are: drink at least 0.79 gallons of water per day. One glass every two hours. Masturbate at least two times a week. Avoid sitting for a long duration. It puts pressure on your perineum. Regular exercise at three to four times a week at least.

Medication include, tablet Cranpac D (Hibiscus D and D-mannose) twice a day for 30 days, tablet Econorm 250 mg (Saccharomyces boulardii) twice a day for 30 days, tablet Chymoral forte (Trypsin-chymotrypsin) twice a day for 30 days, and capsule Urimax 0.4 mg (tamsulosin) one at night for 30 days. Kindly remember this medicine makes you lightheaded on the first night. So please walk with support or sit for a while before you get up at night. This does not happen after the second dose. Also take capsule Doxycycline 100 mg twice a day for 30 days. Consult your specialist doctor, discuss with them, and take medications with their consent."
