Hello doctor,

My son is crying at the end of urination from yesterday afternoon. The doctor prescribed Ziminic dry syrup 50 mg/5 ml 7.5 ml two times a day. For pain, Cyclopam 3 ml syrup has been given two times till now. He is afraid to pass urine saying it will pain. When he passes urine towards the end, he is crying with pain and stops the urination suddenly. And he goes to pass urine again. The same thing continues again. What can be done to reduce pain? Urine test result shows pus cells: plenty, epithelial cells: occasional, RBC: 6 to 8, bacteria: +, Alb: trace.

It seems to be urinary tract infection from the case history provided by you. You are already being treated with antibiotics and painkiller. You may also give syrup Crocin (Paracetamol) DS 5 ml SOS for pain relief. Urinary tract infection takes time on antibiotics to respond. If the pain is severe, the child may be admitted to start IV antibiotics for early response.

The Probable causes

Urinary tract Infection.

Investigations to be done

CBC, urine routine microscopy and culture, ultrasound KUB.

Differential diagnosis

UTI.

Probable diagnosis

UTI.

Treatment plan

Continue oral antibiotics and painkiller, increase water intake. If the pain is severe and there is difficulty in urination, the child must be admitted for IV antibiotics and further investigating other causes.

Preventive measures

Drink more water.

He is 3 years and 4 months old (36.38 lbs). Yesterday evening temperature: 99.5, Calpol 250 5 ml was given. After that no fever.

You can use the same dose to relieve pain as well. Crocin DS and Calpol 250 are same. Get urine culture sensitivity report based on which antibiotics may be upgraded.

For pain, are both Cyclopam and Crocin needed? Is 0.26 gallons of water and 8.3 oz milk enough fluid intake for a day for my son? We are giving Muout powder for constipation daily.

Crocin would be required to relieve local pain, while Cyclopam will relieve abdominal pain. Yes, 1 to 1.5 liter of water would be sufficient. You can continue Muout (Polyethylene glycol), if the child has constipation.
