Hello doctor,

I am 19 years old in good health. For about a year, I have had some strange heart palpitations. They tend to happen at night when I lie down. They are very strong beats and irregular. It lasts only a couple of minutes but is quite scary as it feels like my heart is beating out of control and out of rhythm. During this, my chest also feels quite tight and uncomfortable. I recently had a physical examination at the university, and the nurse noted that my pulse was a bit irregular. No one in my family has any heart problems. I have anxiety, and I am taking the anti-depressant Fluoxetine 40 mg daily. Kindly help.

Palpitations are very common, and usually, they are not serious until there are accompanying symptoms like lightheadedness, dizziness, fainting or drop attacks, etc. One of the most common causes of palpitations is anxiety, but it can also be due to heart rhythm problems. So, these must be excluded first. I advise you to get an electrocardiogram (ECG) done at the time of palpitations. If that is not possible, get a baseline ECG at any time of day or night and another test called 24 to 48 hours ambulatory ECG. It will record your ECG continuously for 24 to 48 hours, and any palpitation episodes will be recorded. No need to worry; palpitations are not life-threatening. I suggest the following. Decrease tea, coffee, and caffeine intake. Avoid alcohol and smoking. Do regular exercise. Avoid a sedentary lifestyle. Avoid junk food and soda. Follow-up is advised within seven days with these reports. Regards.
