Hi doctor,

I am having really bad anxiety. I have a feeling of hopelessness and fear of the unknown. I have been diagnosed with depression when I was younger and have been to a mental institute once. I just feel like being around no one. I am irritable and have tremors. It has been happening about three times a week. I do not feel happy, always tired, dizzy and almost passed out. My heart is beating funny and barely can breathe. I am a little nervous about going to see my doctor. I fear that he might think I am lying or something.

I understand your concerns. You are describing episodes of panic attacks in which one feels intense anxiety, fear of impending doom, racing or sinking heart, dizziness, breathlessness, numbness, dry mouth, tremors, etc. Mostly, panic attacks are part of psychological origin, but sometimes have some medical underpinnings like hyperthyroidism, hypoglycemia, asthma, angina, etc. So, some basic investigations may be prescribed by your doctor to rule out the same. Once diagnosed with panic attacks of psychological origin, the first thing is to be relaxed that you are not harboring any serious medical condition and apart from anxiety nothing else serious is actually going to happen. Deep breathing exercises and progressive muscle relaxation exercises are helpful. Medical treatment constitutes a prescription of antidepressants like Escitalopram or Paroxetine along with some supportive medication like Clonazepam, Alprazolam and Propranolol. In addition, if you are experiencing persistent hopelessness, then may be a depressive disorder is co-existing, which would also be taken care of similar medicines and therapy. Do not lose hope. It is just part of the illness and you will overcome it. Take care.

Investigation to be done ECG (echocardiogram), thyroid function test, and fasting blood sugar.

Probable diagnosis

The probable diagnosis is panic disorder with depression.
