I notice tiny white spots on my shaft and penis head. It does not spread, and there is no pain, irritation, or itching. I tried a lot of home remedies, but nothing helped. I am under medication for fungal infections on my back. Kindly help.

I understand your concern. Kindly answer me the following to help you better. Does the lesion show a pearly appearance? Do you have any history of injury or microtrauma? Did you had a zip fastener injury? Any rough handling of your penis? Is there any excess scratching and rubbing of lesions? Do you have any swelling? Are you sexually active? Did you have unprotected sex in the last three months? Also, attach the clinical picture of the lesion. Thanks.

Hello doctor,

Thanks for the reply. The answers are as follow. Yes, the lesion is pearly in appearance. There is no significant injury, but the skin got slightly damaged due to rubbing. No zip fastener injury. Yes, there was a rough handling three to four days before these spots appeared. I did rubbing with a ring on my finger; damaged the skin but no wound. There is no swelling. Thanks.

It is balanitis, a fungal infection of the penis. I suggest you apply Candid cream (Clotrimazole) at night for two weeks and Fucidin cream (Fusidic acid) at daytime for two weeks. Take tablet Fluconazole 150 mg once daily after food for three days. Kindly maintain proper genital hygiene. Wash genitals daily with a mild cleanser like Cetaphil (Glycerin, Cetearyl alcohol, Panthenol, Niacinamide, Pantolactone, Sodium benzoate, Citric acid, and water) for 15 days. Please avoid scratching or rubbing of lesions as it may cause secondary bacterial infection. Reduce frequency of masturbation to once weekly. Regards.
