Hello doctor,

I am 25 years old. Sometimes back, I had a different feeling in my duct. I would feel like scratching the bottom part of my penis all the time. It was like a funny itching, not a painful one from the tip to the groin area but just inside the tract. But then the feeling faded away after sometime like a week maybe, this happened a month ago. Nowadays, I notice some stain in my boxers. When I attend to the toilet, I realize there is some stain on the boxer from the penis. I do not know the problem, but I am sure I do not feel fine. Please help.

I am concerned about your complaints. According to your complaints, it may be due to a fungal infection. I can help you with medication. I need to know more about your issue. Are you taking any tablets currently? Is there any discharge from the penis or sides of the penis? Does your mother or father have BP or sugar?

You can take tablet Allegra 120 mg for five days in the morning and night after food, and tablet Candy 400 mg for five days in the morning and night after food. Ointment Lobet GM for local application thrice a day at any time.

I am not taking any medication currently. The discharge is from inside the penis only since it is found around where the tip of the penis rests on the boxer. My dad had a blood pressure.

If it is coming form inside, we need to think of infection.

Di you had any unprotected sex? If yes, is it with a known or unknown person?

I will send you the new medication. Also, it would help if you did some investigations which I mentioned below. You can do the investigations and start medication, and review with reports.

The Probable causes

UTI (urinary tract infection). STI (sexually transmitted infection).

Investigations to be done

TPHA (Treponema Pallidum Hemagglutination test). Herpes simplex virus (HSV) 1 and 2 IgG, IgM. Serum chlamydia trachomatis IgG. HIV 1 and 2 antibodies screening test.

Differential diagnosis

UTI.

Probable diagnosis

UTI.

Treatment plan

Syrup Citralka 5 ml in 100 ml water three times a day after food, tablet Uribid 100 mg twice a day after food for five days, tablet Zerodol P (Aceclofenac and Paracetamol) twice a day after food for three days, and Pan 40 twice a day before food for three days.

Preventive measures

Take adequate water and maintain proper hygiene.

Regarding follow up

Review with reports.
