Hello doctor,

I have a cheese-like layer forming in the foreskin of the penis with a slight itchy feel. I have a slight strain in the groin area near the testicles but not regularly. Kindly advice. Earlier I took Levocet and Flucos tablet for 10 days and applied Candid B cream along with it. I also took Allegra tablet for five days, but the itchiness is there. Now I am taking homeopathy medicine for nearly 10 days and applying Onabet cream for the last couple of days. But after applying Onabet cream itchiness increased.

Attach the picture of the lesion for details. Any history of tight foreskin? Excess scratching and rubbing habit? Scented soap or lotion use? Any irritant chemical contact? Burning micturition? Increase in frequency? Urethral discharge? Burning pain or irritation? Oozing? Fissures? Improper penile hygiene? History of eczema, diabetes, psoriasis, or infection in the past? Swelling, inflammation, or redness of penis?

No history of tight foreskin but last week it was there after I applied Pongamia pinnata fruit oil (karwan ka tel) for three to four days. Then I stopped its use and applying Onabet cream since then and its fine now. No scratching and rubbing habit. Using Patanjali soap has some relief from itching. No history of eczema, diabetes, and psoriasis. Some swelling in testicles is there with slight pain and feeling nausea.

Is there a history of vigorous sex or masturbation? Is there testicular pain when testes held up? Torsion in testes? If there is pain, consult a surgeon. Take tablet Zifi 200 mg twice daily for five days, Emanzen D twice daily for three days, and Candid cream at night for seven days. Maintain proper genital hygiene.

No vigorous sex and masturbation. Not much pain when testes held up and no torsion in testes. There is some relief in itching but not fully.

Apply Fucidin cream at daytime for seven days, tablet Widcet 5 mg twice daily (if itching). It could be a fungal infection due to wet inners, moist, and humid environment (favors growth of fungus). In the past, were the lesions inflammatory, red and now hyperpigmentation?
