Hello doctor,

I used a steroid cream that contains Clotrimazole and Betamethasone on the bottom of the penis head and foreskin. One year later penis is still discolored (sometimes a bit blue at the base - but it is usually a bit red, and the foreskin is red and irritated). What could be the reason?

I shared the picture after retracing the foreskin. I was lathering on the steroid cream in these areas (skin around the head and bottom of the penis head), so it does seem to be from the steroid cream. I had undergone STI tests and the results were fine.

Never use steroid-containing creams, as they will encourage the growth of fungus. I suggest you to follow the instructions mentioned below:

1. Apply Clotrimazole (antifungal) lotion on the affected areas daily.

2. Clean the glans penis with warm salt water daily.

Clotrimazole is not a steroid, but Betamethasone is a steroid. If lotion is unavailable, I suggest you to use the cream once daily after a bath with warm water and dry it under the fan.

So have a warm bath or shower and dry using a fan or apply cream and then dry cream with a fan? So, no need to use warm salt water, as mentioned before? Kindly help.

The lotion is better than cream. If you can use warm salt water to clean it twice daily, that is best but do not add excess salt to the water. After cleaning with warm salt water, allow it dry. After some time, apply Clotrimazole lotion. A bath is unnecessary if you use warm salt water to clean it.

I now got Candix lotion (which looks like a liquid), with every 100 ml containing 1 g of Clotrimazole. Is this fine? I also just got some iodized refined salt.

Could you reconfirm the steps for usage, please?

Also, why do you believe this will cure the problem? Even the last doctor took a sample a couple of weeks ago to see if I had a fungal infection, but the reports were normal.

I will try anyway, as no steroid means I will not have an adverse effect.

Using a cream of 100 ml containing 1 gm of Clotrimazole is fine. Also, do not use iodized salt. Use only ordinary salt.

I assure you that it is not a steroid.

I suggest you to follow my advice and use it for five days.

What is regular salt? I have had warm showers and then used the lotion. There seems to be a mild improvement. Sometimes, it appears natural or reddish in color and sometimes retains bluish color. The foreskin seems less inflamed than before.

Kindly ensure you are not a diabetic and get a fasting blood glucose test done. I am sure you will improve. Fungal infections are slow to clear, so let us wait for at least 20 days. If required, I will suggest oral antifungal medicines.

I am sure I underwent a blood sugar test last year, and the reports were normal. But I will do one again. If the levels are high, does that mean I have to reduce my sugar intake? However, it seems unlikely as I am in good shape and 27. It is strange how the blueness returns and sometimes goes away to a more natural, red, or pinkish shade. But some days, they seem the same as the original picture. Is it not possible it is related to the skin being thinned or some other issue? Has it got something to do with the blue arteries in that region? However, it is just a fungal and will go away in 20 days or so.

It is common for people with diabetes to get fungal infections frequently. That is why I am suggesting you rule out diabetes. Thinning of the skin is a side effect of prolonged use of Betamethasone.

Kindly understand that the medication takes time to prevent the growth of fungus. There was some thinning of mucosa due to the use of steroid cream. That is the reason for using topical antifungals, Vitamin B complex, and Vitamin C supplements, to aid faster healing.

The other issue is that the bottom of the glans is now swollen (I have attached the image). Is it likely from the usage of the steroid cream? Will the current treatment aid with the glans, or is the glans base swelling a separate problem?

What is strange is that the first drop of the lotion seemed to make the glans retract to the original size. Since I have been using it regularly, it seems to have retracted to the original size. However, it is still swollen and looks damaged at the bottom. That is one of the main issues I have. But yes, I will continue the medication course. Today is the last day of two drops of the lotion. I have been taking tablets of vitamin B complex and vitamin C, also.

I used a small tube, which must have been a couple of weeks of applying. As you can see from the image, the base of the glans appears there is some irritation, dryness, redness, and sometimes bluish discoloration (but I never saw this before steroid usage)

And the foreskin around the base of the glans looks strange instead of its natural appearance. Could it be because the skin was destroyed by steroid usage? Before, my penis looked normal. But now it appears light pink or reddish and it looks different.

It should heal gradually if there is no itching, redness, etc. Also, get your fasting blood glucose test done to rule out diabetes. And continue using the Clotrimazole lotion every day.

I do not have itching, but there is redness on the glans and skin surrounding it. I apply the lotion at night. Also, I do warm salt water washes in the morning. In addition, I do not eat sugary items (other than rice). And I think I had a glucose test within the last year or two, and it was fine.

I guess there are around eight days left to use the lotion. I have to retract and use one drop of the lotion, right, doctor?

3. Take multivitamin supplements that contain vitamin B-complex, vitamin C, and zinc. Take one capsule daily for 20 days.

4. Drink adequate water, at least 1.5 liters daily.
